Towel Clip Reduction of the Depressed Zygomatic Arch Fracture

As fraturas da região do complexo zigomático orbital envolvem estruturas anatômicas nobres. O diagnóstico dessas fraturas é baseado em achados clínicos, assim como exames complementares por imagem, apresentando papel muito importante, seja para definir a extensão das fraturas, bem como para estabelecer o planejamento do tratamento. No presente trabalho, é apresentado um relato de paciente de 24 anos, gênero masculino, vítima de acidente motociclístico, com trauma de face, apresentando edema aparente em região periocular, equimose periorbitária, hemorragia subconjuntival e afundamento do complexo zigomáticoorbitário esquerdos, causando assimetria facial. O diagnóstico clínico e imaginológico foi de fratura do complexo zigomático orbital a esquerda. Optou-se pelo tratamento cirúrgico com redução e fixação das fraturas mencionadas com bom posicionamento do corpo do osso zigomático, assim como do arco zigomático também fraturado. Num pós-operatório tardio é possível notar a estabilidade do tratamento cirúrgico cruento, seguido de uma boa e quase imperceptível cicatrização dos acessos cirúrgicos aqui preconizados.Descritores: Traumatismos Faciais; Diagnóstico; Terapêutica; Fraturas Orbitárias.

Introduction:A zygomatic complex fracture includes disruption of the four articulating sutures: zygomaticofrontal, zygomaticotemporal, zygomaticomaxillary and zygomaticosphenoidal sutures. All zygomatic complex fractures involve the orbital floor and therefore an understanding of orbital anatomic features is essential for those treating these injuries.Aims and Objectives:To analyze the efficacy and shortcomings of this approach. To evaluate the adequacy, role of tarsorrhaphy, difficulties, role of steroid in postoperative edema control in lower lid blepharoplasty approach.Materials and Methods:A total number of six patients were included in this study and all the patients were treated surgically under general anesthesia. All the patients were approached through lower eyelid blepharoplasty incision. The first skin crease in the lower eyelid region is selected for this incision.Results:All patients were administered with steroid injection. Frost sutures were placed in four cases and tarsorrhaphy was done in two patients. Three cases encountered immediate mild edema and immediate scar formation. Late scar was present only in two patients with a follow up of three months.Conclusion:Lower eyelid blepharoplasty incision is an excellent, non complicated, simple procedure in the management of fractures in the infraorbital region, orbital floor, which occurs as a part of zygomatic complex fractures.

The face symmetry has prominent role in the human body after injuries and accident. The zygomatic region is important factor in the injuries face. Due to its location, its fracture is the 2nd frequent fractured bone of mid-facial. Zygomatic bone fractures are more abundant in young males and its incidence and etiology is different based on location. Post-traumatic facial deformity is the most incorrect reconstruction of the facial skeleton. Inadequate healing of the supported soft tissues lead to malposition of landmarks, shrinkage and thickening. The zygomatic bone fracture and coronoid process impingement lead to restricted mouth opening. Interruption in zygomatic position has psychological, aesthetic and functional effects which impairs the function of mandible and ocular tissue. Therefore, diagnose and properly management of the zygomatic bone injury is important. Healing displaced fragments of zygomatic bone after inadequate fixation and reduction of fracture consequences facial asymmetry. There is a lack of information about evaluation of benefits and costs of different zmc fracture methods. So, this literature review was done to characterize the etiology, incidence, clinical findings and results of different treatment trends of zygomaticomaxillary complex fractures. Surgeons have been utilized numerous approaches, but there are different ideas for the best one.
