Toxic epidermal necrolysis and Stevens-Johnson syndrome in South Africa: a 3-year prospective study

Abstract: Most patients with TEN/SJS were HIV positive and female. We validated the use of SCORTEN and identified several prognostic indicators, most significant being HIV-tuberculosis co-infection, ≥40% skin involvement and severe sepsis.

“…the Severity-of-Illness Score for Toxic Epidermal Necrolysis (SCORTEN), which was recently validated at our institution. [12] Our data suggest that SCCOR-TB cutoffs of both ≥2 and ≥3 were associated with significant increments in mortality, which could potentially guide clinicians in identifying patients at a significantly increased risk of dying from TB in the ICU. We specifically aimed to identify parameters that could be readily assessed at practically all levels of medical care, and used both the prospective data set in addition to known poor prognostic factors (e.g.…”

“…Observed proportion of HIV-positive patients in the present study is lower than South Africa (78.67%), Togo (54.6%) and higher than France (7.3%). [28,45,46] Incidence rate of SJS is 1,000 fold higher in HIV-positive patients as compared to general population in Germany. [47] In contrast to our study, higher cases of malignancy (10.6%) than HIV infection (6.6%) were reported in EuroSCAR.…”

“…These epidermal detachments are considered life-threatening (2) and are most frequently manifested by adverse drug reaction. In addition to occurring as a reaction to drugs, these skin eruptions are also associated with underlying infectious diseases, and include the cutaneous manifestations of disseminated candidiasis (3,4), Mycoplasma pneumoniae (5), Chlamydia pneumoniae (6), cytomegalovirus infection (7) and human immunodeficiency virus (8,9). The mortality rates of these skin eruptions have been reported to range from 16 to 25% (10)(11)(12)(13).…”
