Results: The surgical procedures were: plasty (n=20) or mitral valve replacement (n=36), aortic valve replacement (n=14), atrioseptoplasty using pericardial patch (n=32), tricuspid valve repair with rigid ring (n=6) and surgical correction of atrial fibrillation with radiofrequency (n=12). There were no complications during the procedures. There was no conversion to thoracotomy in neither case. Two patients developed atrial fibrillation in the postoperative period. There was an episode of stroke seven days after the hospital discharge and one death (0,9%) due to systemic inflammatory response syndrome (SIRS).Conclusion: This study demonstrates the coverage of pathologies that are possible to be approached by videoassisted cardiac surgery with cardiopulmonary bypass being a safe and effective procedure with low morbimortality. Minimally invasive video-assisted cardiac surgery is already a reality in Brazil, demonstrating excellent aesthetic and functional results. Rev Bras Cir Cardiovasc 2009; 24(3): 318-326 and transesophageal echocardiogram, ultrasound of the carotid arteries, abdominal, iliac and femoral arteries. In patients with suspicion of some obstruction of peripheral arteries, angiotomography of the thoracoabdominal aorta was requested. Special care was taken at the time of cardiac catheterization, avoiding puncturing the femoral vessels, giving preference to puncture of the radial artery and complete aortography was performed during the exam. After the first 40 cases, we included patients with an ejection fraction below 55%, emergencies, reoperations and obese patients.The age ranged from 18 to 68 years, and 57 patients were female (55.8%).

Background: Gynecomastia is a benign enlargement of the male breast usually bilateral sometimes unilateral resulting from proliferation of glandular component of the breast. It is defined clinically by presence of rubbery or firm mass extending from nipple. The glandular tissue grows under influence of hormonal stimulation and is tender. Gynecomastia frequently presents social. Psychological, difficulties as low esteem and shame to sufferer. During adolescence many young males have gynecomastia and they are eager to do surgery of gynecomastia. Aims and objectives of the study was to correct deformity restoring normal contour to the chest, maintaining viability of nipple and areola. Also avoiding excess scarring and preventing saucer type deformity. To relieve emotional discomfort, psychological distress, and intolerable pain, to relieve shame in going to society, social gathering even doing marriage. To study effect of adding liposuction to surgical excision. Methods: This is two-year study of 20 cases of gynecomastia. Clinical and Laboratory findings were normal. preoperatively patients are selected by their complaints, discomfort, psychological effects, shame, depression, anxiety and size of gynecomastia. In surgery, we have done is liposuction thoroughly after infiltration with adequate amount of ringer solution and Inj adrenaline 1:100000 concentration. In gynecomastia with group 1 and 2 we used websters incision, in group 2b we used extended websters incision if required. In very large gynecomastia with skin excess we have done breast reduction with liposuction and free nipple grafting in one case and medial pedicle based, superiorly based flap in two cases, two cases with circumareolar skin excision and liposuction in group 2 b case. In rest 16 cases we have done liposuction, excision through websters incision. Results: In our study of 20 cases done in two years, in our department of plastic surgery at NKPSIMS, one was unilateral and rest 19 were bilateral gynecomastia cases. In all cases liposuction as treatment modality used and has given satisfactory outcome in 18 (90%) cases out of 20, 3 (10%) cases want more liposuction and if possible re excision. Average hospital stay was 2 days. Post-operative recovery was good in majority cases but in 2 ((10%) cases post op numbness and ischemia at margin of areola occurred treated conservatively. In one case (5%). dehiscence of wound healed Conservatively. All cases of breast reduction were healed well. All cases benefited psychologically by surgery and their self-image in society improved lot. Conclusions: The problem of excessive fat and fibroglandular tissue is managed by liposuction and excision through websters incision. In high grade gynecomastia of grade 3 we have done breast reduction. This has corrected deformity, restoring normal contour to majority of patients and they improved psychologically, and their self-image improved and so their social life.

81Avaliação da qualidade de cicatrizes em mamoplastia de aumento por via submamária e transareolomamilar Avaliação da qualidade de cicatrizes em mamoplastia de aumento por via submamária e transareolomamilar: um estudo longitudinal prospectivo Scar quality evaluation following transareolomamilar and submammary augmentation mammaplasty: a prospective longitudinal study RESUMO Introdução: O aumento das mamas se tornou um dos procedimentos mais realizados em Cirurgia Plástica. A cirurgia de aumento dos seios é quase inteiramente determinada por três variáveis: localização da incisão, plano de alocação da prótese e tipo de implante. Não há provas incontestáveis que apóiem a superioridade de uma combinação de opções sobre outra. O objetivo deste estudo é comparar, através da análise do cirurgião e das pacientes submetidas ao procedimento, a qualidade da cicatriz originada pela cirurgia de aumento mamário utilizando a via submamária e transareolomamilar. Método: Estudo longitudinal prospectivo para investigar pacientes submetidas a aumento mamário estético. Foi utilizada a Escala Paciente e Observador de Avaliação da Cicatriz para mensuração da qualidade cicatricial na revisão de um ano. Resultados: Quarenta e nove mulheres foram submetidas a mamoplastia de aumento primária por via inframamária (22 pessoas) ou abordagem transareolomamilar (27 pessoas), em um seguimento médio de 13 meses. O grupo transareolomamilar apresentou melhores resultados na avaliação do paciente. Entre os cirurgiões, a avaliação de ambos os grupos apresentou resultados semelhantes. Conclusões: Concluímos, com base nas escalas utilizadas e nos dados clínicos obtidos, que a via transareolomamilar demonstrou qualidade superior na avaliação de pacientes. Não houve diferença estatisticamente significativa entre as duas incisões na análise dos cirurgiões. Estudos adicionais envolvendo maior número de pacientes são necessários para comparar o resultado de cada uma das incisões avaliadas.Descritores: Mamoplastia. Implantes de mama. Implante mamário. The surgery of breast augmentation is almost entirely determined by three variables: selection of incision location, pocket plane for implant placement, and appropriate implant. There is no incontrovertible evidence that supports the superiority of one combination of choices over another. The aim of this study is compare the scar quality by means of patient and surgeon opinion, in women who have undergone augmentation mammaplasty by either the inframammary or transareomamilar approach.
