Transepidermal elimination of urate-like crystals: a new perforating disorder?

Abstract: We report two men who developed a transient perforating disorder characterized by transepidermal elimination of negatively birefringent needle-shaped crystals similar to monosodium urate. This striking clinical presentation has not previously been described and we propose that it be added to the group of diseases known as the primary perforating disorders.

“…A unique crystalline perforating disease was first described in 1999 by Lucke et al 1 Kossard et al 2 suggested the term necrotizing infundibular crystalline folliculitis (NICF) because they felt it represented a form of crystalline folliculocentric necrosis rather than a perforating disorder. Patients present with waxy papules across the forehead or back.…”

“…In the cases described by Lucke et al, 1 needle-shaped crystals within the filamentous material were identified. Although several hypotheses for the development of NICF have been proposed, 1,2 its pathogenesis, clinicopathological spectrum, and course remain to be elucidated.…”

“…T he microscopic findings of necrotizing infundibular crystalline folliculitis (NICF) were first described by Lucke et al 1 This disorder is characterized clinically by sharply demarcated waxy papules and histologically by distinctive filamentous material within the partly necrotic follicular ostium and infundibulum. In the cases described by Lucke et al, 1 needle-shaped crystals within the filamentous material were identified.…”
