Transfusion of Red Cells in Hematopoietic Stem Cell Transplantation (TRIST Study): A Randomized Controlled Trial Evaluating 2 Red Cell Transfusion Thresholds

Abstract: Background Evidence regarding red blood cell (RBC) transfusion practices in Hematopoietic Stem Cell Transplantation (HSCT) is lacking. As a result, the impact of RBC transfusions on outcomes following HSCT is not well understood. Furthermore, studies have demonstrated that a restrictive RBC transfusion threshold (70 g/L) was noninferior to a liberal threshold (≥80–90 g/L) in terms of mortality, length of hospital stay, and quality‐of‐life measures, among other clinically important indicators in patients with hematologic malignancies undergoing treatment . “…There was 83% strict adherence (81% acute leukemia subgroup, 85% ASCT subgroup) to the CPOE transfusion parameters. This is similar to the adherence rate reported by Berger et al when their institution switched from a double to single RBC unit transfusion policy for leukemia and stem cell transplantation patients . The slightly higher compliance rate in the ASCT subgroup may be due to the lower transfusion needs of this population in general.…”
