Transfusion‐related acute lung injury prevention measures and their impact at Canadian Blood Services

Abstract: Hemovigilance data demonstrated an initial increase in TRALI cases, likely due to increased adverse event reporting and awareness of TRALI, followed by a decrease in cases related to all components. TRALI prevention measures and possibly the switch to the buffy coat production method may have contributed to the decline. Classification of cases remains challenging.

“…The proposed mechanism of TRALI involves antibodies from donor blood components that are directed against Human Leucocyte Antigens (HLA). These antibodies are most numerous in women who have been pregnant, hence the risk of TRALI can be reduced by using predominantly male plasma [40]. …”

“…Es más frecuente tras la administración de PF pero también se relaciona con los CP, que están vehiculizados con plasma 136 . La utilización de plasma derivado de donaciones de sangre de varones, el empleo de menores volúmenes de plasma en los concentrados y la utilización de donaciones únicas frente a pooles reduce el riesgo de estas complicaciones.…”

“…US proposals highlight the need for a ‘partial, incremental approach in making policy decisions’ and suggested ‘a plasma diversion strategy’, which diverted female plasma for fractionation and used male plasma for transfusion, but which sought to avoid alienating and confusing female donors (Eder et al, 2007). In Canada, a combination of preference for male plasma transfusion and high-volume plasma components, and screening of women who have a history of pregnancy, has been thought to improve transfusion outcomes (Lin et al, 2012). …”
