Transient Ciliochoroidal Detachment After Ab Interno Trabeculotomy for Open-Angle Glaucoma

Abstract: IMPORTANCE Although trabeculotomy is a glaucoma surgical procedure for modest intraocular pressure (IOP) reduction, some eyes exhibit very low IOP during the early postoperative period. To our knowledge, the reason and its effect have not been investigated. OBJECTIVES To investigate ciliochoroidal detachment (CCD) immediately after ab interno trabeculotomy and evaluate its effect on IOP immediately after surgery.

“…Third, although AS-OCTA images were successfully acquired, they still had significant noise. Multiple image averaging is known to be helpful in improving image quality in posterior-segment OCTA 27 and AS-OCT, 43, 44 as well as posterior-segment OCT 45 . AS-OCTA imaging with multiple image averaging might be a possible method to improve the image quality in future studies.…”

“…I morphologically classified the appearance of the trabeculotomy cleft into three groups ( Figure 2 ), and the cleft at the anterior edge of the trabecular meshwork was the most frequent pattern after the incision using a microhook; this should associate with the significant IOP reduction after μ LOT [ 5 , 6 ], although the association between the patterns and surgical efficacy has not been clarified yet. Recently, based on postoperative observation using anterior segment OCT, a possible connection between the anterior chamber and suprachoroidal space through the iris root was reported in cases after trabectome surgery [ 8 ]. Although I did not find evidence of cyclodialysis or dissociation of the pectinate ligaments of the iridocorneal angle in our cases, iOCT can be used to elucidate the mechanism of the phenomenon reported by Akagi et al…”

“…Eyes with ciliochoroidal detachment had lower mean IOP than eyes that did not, but IOP was never <5 mmHg in any eyes. Only four eyes had persistent ciliochoroidal detachment at 1 month, but visual acuity was not reported 32. Osman and AlMobarak described a case of cyclodialysis cleft following AIT with hypotony (IOP <5 mmHg), persisting to the fourth postoperative month at the time of the report 33.…”
