Transient Left Ventricular Apical Ballooning Syndrome: The First Series in Taiwanese Patients

Abstract: Previously thought as exclusive in Japanese patients, cases of transient left ventricular apical ballooning from other countries have also been reported. The cause remains unknown. From January 1997 to December 2005, 25 patients presenting with signs and symptoms of acute myocardial infarction with normal coronary arteries were analyzed. In all, 10 patients fulfilled all the criteria for transient left ventricular apical ballooning. In all, 6 patients had chest pain and diaphoresis, 5 patients had ST segment e… Show more

“…Arrhythmia resulting from QT prolongation is commonly observed in patients with TTS. The prevalence of QT interval prolongation among TTS patients is high, ranging from 50% to 100% according to different case series [40], [41], [42]. QT interval prolongation might precipitate TdP, a polymorphic ventricular tachycardia that might lead to ventricular fibrillation and sudden death.…”

“…Apical ballooning is a distinct entity and is usually not observed in acute coronary syndrome [16,26]. Coronary artery spasm and left ventricular outflow obstruction have been observed in a few cases, but reports are inconsistent [30].…”

“…since the syndrome is more commonly seen in postmenopausal women, estrogen may play a role. 2,4,5,[15][16][17] other proposed mechanisms are elevated catecholamines that cause cardiotoxicity and coronary artery spasm, which lead to myocardial stunning. 3-5, 15, 17…”

“…during stressful situations, the increase in catecholamines may cause lvot obstruction that leads to ischemia, in turn causing regional wall motion abnormalities and release of cardiac enzymes. 17…”
