Transient Respiratory Difficulty following Cesarian Delivery

Abstract: 57 newborn infants delivered by planned, repeat cesarian section were studied to determine the role of surfactant in transient neonatal respiratory distress. 22.8% of the newborn infants studied had transient tachypnea of the newborn. The mean amniotic fluid lecithin-sphingomyelin ratio (L/S) was 2.8 in normal infants and 2.6 in infants with transient tachypnea. The mean gastric aspirate L/S at the time of delivery was 3.0 in the normal infants and 2.7 in infants with transient tachypnea. There were no statist… Show more

“…However, others did not support this. 20 TTN was strongly associated with lower GA, whereas the association to birthweight was probably related to lower GA. In Israel in general, and specifically in our population, dating of GA is very accurate, because most pregnant women are closely observed by obstetricians from early weeks of gestation and multiple ultrasonographic evaluations are readily available.…”

“…18 Transient tachypnea was defined as grunting, retractions, respiratory rate > 60/min for more than 6 hr after birth, without oxygen requirements. 19 Hypoglycemia was defined as a plasma glucose concentration < 30 mg/dl in the first 4 hr of life. 20…”

“…[6][7][8][9][10][11] TTN results from delayed clearance of fetal lung fluid at birth 12,13 and occurs in isolation or in association with other neonatal respiratory illnesses, such as surfactant deficiency, 14 neonatal pneumonia 14 and persistent pulmonary hypertension of the newborn. 15,16 While TTN is a self-limiting condition, affected infants often undergo evaluation with laboratory testing, chest radiography and close monitoring in a special care nursery (SCN) or neonatal intensive care unit (NICU).…”
