Transmesocolic hernia with strangulation in a patient without surgical history: Case report

Abstract: Transmesenteric hernias have bimodal distribution and occur in both pediatric and adult patients. In the adult population, the cause is iatrogenic, traumatic, or inflammatory. We report a case of transmesocolic hernia in an elderly person without any preoperative history. An 84-year-old Korean female was admitted with mid-abdominal pain and distension for 1 d. On abdominal computed tomography, we diagnosed transmesocolic hernia with strangulated small bowel obstruction, and performed emergency surgery. The pos… Show more

“…IHs run a risk of strangulation and ischemia, which can be life-threatening. This risk is particularly profound in transmesocolic hernias, which account for up to 40% of all IHs strangulation [ 10 ]. The diagnosis of IH is dependent on CT imaging.…”

“…Certain CT features represented by clustering of dilated small bowel peripherally without overlying Omentum and leading to colonic displacement centrally may be suggestive of transmesenteric hernias [8]. However, final diagnosis will only be confidently achieved intraoperatively [3, 9].…”

“…An early intervention is recommended to prevent unnecessary bowel resections and even death [1, 3, 8]. Our patient was operated on within 1 h of transfer to our centre and thus we prevented ischemia and avoided bowel resection.…”
