Transmission of HIV-1 infection due to a fist fight

Abstract: We report a case of HIV-1 infection transmission caused by a fist fight between brothers. A 30-year-old Caucasian UK resident man developed 'flu-like illness with symptoms of lethargy and weakness. Persistent lymphadenopathy six months later lead to HIV antibody testing, which was positive. Of note, his 37-year-old brother, who was HIV antibody-positive since August 2000, was taking HAART (combivir and nevaripine) with CD4 350 x 106/L (16%) with viral load 4800 copies/mL (log 2.58). A bloody fight had occurred… Show more

“…Although less attention has been paid to other blood exposures, some studies have, for example, linked HIV infections to tattooing 18,19 and fighting. 20 Even without further investigation, the information from this study motivates several recommendations to strengthen HIV prevention. Specifically, public education could help people to avoid HIV infections by telling people how long HIV survives in trace amounts of blood outside the body and how to clean instruments, and by warning people to avoid unnecessary injections and infusions.…”

“…Although violent encounters, particularly fistfights, involving HIV positive individuals have previously been identified as a mode of HIV transmission [ 3 , 7 , 8 ], this potential route of transmission has not been commonly reported, and as far we know, has never been reported among IDU. The present case suggests that the potential for HIV exposure should be explored among individuals involved in violent encounters in high HIV prevalence settings, especially among IDU, since the level of violence involved in this case is not unique [ 9 , 10 ].…”

“…Commonly, HIV is transmitted by sexual contact across mucosal surfaces, by sharing of injecting equipment, through contaminated blood transfusions, and by maternal-infant exposure [4]. Nonetheless, numerous unusual transmission incidences such as father to son [5], from one child to another [6], between two adolescent brothers [7], from child to mother [8], between two females [9], due to human bite [10], from a traditional healer to client [11], and via fights [12][13][14] have been documented in the literature. By virtue of HIV's dynamic nature and multiple transmission modalities, it is crucial to establish the likely route of infection if the pandemic is to be fully understood.…”
