Single-port laparoscopic technology is effective in minimally invasive surgery. However, this technology requires expensive instrumentation. In the present study, an alternative minimally invasive technique, periumbilical laparoscopic surgery through triple channels using common instrumentation, is introduced. Increased use of this new technique may be worthwhile since its results are comparable with those of single-port laparoscopic cholecystectomy. Periumbilical laparoscopic cholecystectomy using common instruments through triple channels was performed in 78 cases of simple cholecystolithiasis and 30 of gallbladder polyposis which were compared with a control group consisting of 356 cases of traditional laparoscopic cholecystectomy. The surgery was successfully completed using common instrumentation without complications in 106 cases from the experimental group. However, in 2 cases the surgery was changed to the traditional laparoscopic cholecystectomy due to bleeding in the area of Calot’s Triangle. No statistical differences in the amount of surgical bleeding, intestinal function restoration time, hospitalization time and cost were observed between the two groups. The mean surgery times of the experimental and control groups were 110.31±14.57 and 43.98±7.64 min, respectively. The difference in surgery times was statistically significant. Based on relevant experience of the process of laparoscopic cholecystectomy, the periumbilical triple channel technique is safe and feasible for use in basic-level medical units and does not produce abdominal scarring so an unblemished appearance is preserved. Moreover, this approach only requires common laparoscopic instruments.

Transumbilical laparoscopic cholecystectomy assisted with a second port. Experience in 60 patientsBackground: Transumbilical laparoscopic surgery offers substantial esthetic benefi ts. The use of a second port to introduce a second trocar, reduces the need of special or articulated instruments. Aim: To report the experience with transumbilical cholecystectomy, assisted using a second port, to reduce technical complexity. Patients and Methods: The surgical technique was prospectively evaluated in 60 patients aged 18 to 77 years (39 women), that required a cholecystectomy. All had a surgical risk classifi ed according to the American Society of Anesthesiologists (ASA) as I or II. Patients with gallbladder cancer, choledocholithiasis, were excluded. Two trocar (10 and 5 mm) were introduced by transumbilical incision and other one 5 mm trocar was introduced in the right iliac fossa for 0° frontal optics. Results: The mean operative time was 72 minutes (range 35-165). Fifty seven patients had a 24 h hospital stay. Two patients had to be converted to open surgery due to a complex pedicle dissection and one patient required an additional trocar. There were three complications. An umbilical seroma, a bile leakage through a Luschka duct, that was managed conservatively and one bile tract stenosis, that appeared four months after the operation and required surgical repair. Conclusions: Transumbilical cholecystectomy using a second additional port was a successful procedure, with similar esthetic results as single port surgery, but less costly.

: Although single-incision laparoscopic cholecystectomy is now widely performed in patients with cholecystitis, some cases require an additional port to complete the procedure. In this study, we focused on risk factor of additional port in this surgery. We performed single-incision cholecystectomy in 75 patients with acute cholecystitis or after cholecystitis between 2010 and 2014 at Gunma University Hospital. Surgical indications followed the TG13 guidelines. Our standard procedure for single-incision cholecystectomy routinely uses two needlescopic devices. We used logistic regression analysis to identify the risk factors associated with use of an additional full -size port (5 or 10 mm). Surgical outcome was acceptable without biliary injury. Nine patients (12.0% %) required an additional port, and one patient (1.3% %) required conversion to open cholecystectomy because of severe adhesions around the cystic duct and common bile duct. In multivariate analysis, high C-reactive protein (CRP) values ( 7.0 mg/dl) during cholecystitis attacks were significantly correlated with the need for an additional port (P = 0.009), with a sensitivity of 55.6% %, specificity of 98.5% %, and accuracy of 93.3% %. This study indicated that the severe inflammation indicated by high CRP values during cholecystitis attacks predicts the need for an additional port.
