Tratamento da síndrome do roubo devido a fístula arteriovenosa para hemodiálise em membro inferior por meio da técnica de ligadura arterial e revascularização distal (técnica de DRIL)

Abstract: Arteriovenous fistula with adequate blood flow is extremely important for patients with chronic renal insufficiency undergoing hemodialysis. Steal syndrome is a complication of arteriovenous fistula, but it is rare and its treatment is directly indicated when there are associated symptoms. Although many treatments have been proposed for its repair in the upper limbs, distal revascularization and interval ligation is currently the treatment of choice. This original case report describes the treatment of steal s… Show more

“…In practice, steal may not be suspected post operatively and may only appear when the AVF is used. 12 In the second series published, reported by the same authors in 2005, the incidence of ischemic complications was reduced to zero. 13 This substantial improvement in relation to the first series was attributed to better patient selection, excluding those with significant peripheral arterial occlusive disease (PAOD).…”

“…Apesar de excelentes taxas de perviedade primária e secundária, 73% e 86%, respectivamente, os autores alertaram para o roubo arterial pela FAV como um potencial complicador associado ao procedimento cirúrgico. Na prática, o roubo pode não ser suspeitado no pré-operatório e surgir somente com a utilização da FAV 12 .…”

“…Malgor et al reported this event in the early postoperative period of a straight and synthetic AVF on the right lower limb, with clinical signs such as coldness, recently onset of claudication and significant drop in the ankle-brachial index (ABI), similar to those reported in cases of the upper limbs. The patient was submitted to DRIL, the symptoms disappeared and the access was maintained 13 . The patient may occasionally present paresthesia distal to the anastomosis due to flow decrease of the vasa nervorum, a symptom that tends to be limited 4 .…”
