Abstract:Vitiligo is an acquired, often progressive disorder of hypopigmentation. A lot of research has been conducted during the years yet its exact etiology still remains elusive. As a result even after so many years, there is no ideal treatment for vitiligo. Treatment options such as Narrowband Ultraviolet B (NB-UVB), Targeted Phototherapy, and Excimer laser on the medical front, in addition to epidermal cell transplantation and melanocyte culture transplants on the surgical front, have all revolutionized the management of this psychologically devastating disease.

ÖzetDeğişik dalga boylarıyla farklı tiplerdeki fototerapi cihazları dermatolojik hastalıkların tedavisinde uzun yıllardır kullanılmaktadır. Geniş-bant Ultraviyole B (UVB), darbant Ultraviyole B (dbUVB), Ultraviyole A1 (UVA1), Psoralen-Ultraviyole A (PUVA) (Psoralen+UVA) ve 308 nm excimer lazer dermatolojide kullanılan fototerapi seçenekleridir. Fototerapi cihaz ve sistemleri tedavinin yan etkilerini azaltmak ve etkinliğini artırmak için sürekli gelişmektedir. Dermatolojik hastalığı olan pediatrik hastalarda özellikle dirençli vitiligo, psoriasis ve atopik dermatit tedavilerinde, fototerapi daha sık kullanılmaktadır. Karsinogenez ve fotoyaşlanma gibi uzun dönem yan etkilerden sakınmak için, erişkinlerden farklı olarak çocuklarda dbUVB, UVA1 gibi daha sınırlı dalga boylarını içeren fototerapi yöntemleri tercih edilmektedir. Fototerapinin etkinlik ve güvenirliğinin ortaya konması için standart fototerapi tedavi protokollerinin uygulandığı çalışmalara ihtiyaç vardır. Sum maryDifferent types of phototherapy devices with various wavelengths are used in the treatment of dermatologic diseases for several decades. Broad-band ultraviolet B (UVB), narrowband ultraviolet B (nbUVB), ultraviolet A1 (UVA1), Psoralen+ultraviolet A (PUVA) and 308 nm excimer laser are treatment options of phototherapy used in dermatology. Phototherapy devices and systems are constantly developing to reduce adverse reactions and increase efficiency of the treatment. Phototherapy is being used more frequently in pediatric patients with dermatologic diseases especially in the treatment of refractory vitiligo, psoriasis and atopic dermatitis. Apart from the adult patients, nbUVB and UVA1, representing the more restricted wavelengths of ultraviolet radiation, are preferred for children to avoid long term side effects like carcinogenesis and photoaging. Targeted Broadband UVB Phototherapy forthe Treatment of Localized Vitiligo

Background: Several new treatments appear to have higher success rates than previous therapies for the treatment of vitiligo, and targeted phototherapy is an alternative that may prove to be time-efficient and an effective therapeutic option for the management of this condition.Objective: The aim of this study was to evaluate the efficacy of targeted broadband UVB phototherapy for the treatment of localized vitiligo.Methods: Fifty-three vitiliginous patches on thirty-five patients (16 males, 19 females) were treated using a targeted broadband UVB phototherapy device. Treatment was started at 70% of the minimal erythema dose, and then gradually increased. Lesions were treated twice a week for a maximum of 60 treatment sessions.Results: Subjects tolerated treatment well. Forty-one of the fifty-three patches (77.4%) achieved repigmentation on ≥75% of the treated areas. Best results were obtained on the face and neck; 20 of the 24 patches (83.3%) concerned achieved ≥75% repigmentation. The least response was on the hands and feet; 3 of 6 patches (50%) showed ≥75% repigmentation. The results were better for focal type than segmental type vitiligo.Conclusion: Targeted broadband UVB phototherapy appears to be highly effective at restoring pigmentation in patients with localized vitiligo.
