Treatment of postoperative pain after total hip arthroplasty: comparison between metamizol and paracetamol as adjunctive to opioid analgesics—prospective, double-blind, randomised study

Abstract: In this study, we have also shown excellent efficacy of paracetamol and metamizole combined with opioids, but metamizole proved to be a better analgesic than paracetamol. It is also necessary to mention the financial aspect considering that intravenous paracetamol is about ten times more expensive than an equivalent analgesic doses of intravenous metamizole.

“…The rest of the sample studied had moderate to severe pain, so they merited reinforcements analgesic morphine 0.4 mg / kg, of which most adequately responded to the administration of a single dose, this data also showed no statistically significant differences between individuals who were given paracetamol or ketorolac and match various studies that have compared the use of paracetamol, desketoprofeno, ketorolac and metamizol lornoxicam, among other NSAIDs and COX2; that have demonstrated the decreased consumption of morphine for postoperative pain management [27][28][29][30][31].…”

“…For the other competitor, paracetamol, a drug being considered as harmless over decades, safety is now also questioned by studies reporting a paracetamol‐induced COX‐2‐inhibition resulting in hypertension, cardio vascular events and bleeding disorders, specifically in the elderly (Chan et al., ; Hinz et al., ; Rahme et al., ; Sudano et al., ). Additionally, several studies indicate its low analgesic potency (Nabal et al., ; Oreskovic et al., ; Williams et al., ; da Costa et al., ). These recent results have apparently not been translated into clinical practice yet, as paracetamol is still more frequently used intra‐ and postoperatively than NSAIDs and COX‐2 inhibitors.…”

“…Nach Ansicht der Expertengruppe kann Metamizol wie die anderen NOPA in der Akutschmerztherapie als Medikament der ersten Wahl eingestuft werden und ist hinsichtlich analgetischer Potenz wahrscheinlich vergleichbar mit NSAIDs, auch wenn die Studienlage nicht so umfangreich ist [ 45 , 60 , 64 , 82 , 91 , 92 ].…”
