Treatment of severe mitral regurgitation caused by lesions in both leaflets using multiple mitral valve plasty techniques in a small dog

Abstract: Mitral valve plasty (MVP) is preferred over mitral valve replacement (MVR) for mitral regurgitation in humans because of its favorable effect on quality of life. In small dogs, it is difficult to repair multiple lesions in both leaflets using MVP. Herein, we report a case of severe mitral regurgitation caused by multiple severe lesions in the posterior leaflet (PL) in a mixed Chihuahua. Initially, we had planned MVR with an artificial valve. However, MVP combined with artificial chordal reconstruction of both … Show more

“…Additionally, the cardioplegic solution volume was decreased to half of the usual volume (20 ml/kg) to avoid more hemodilution, although we used the lowest deep HT (ET 20°C) for cardioplegia. Thereafter, we used a hemoconcentrator to prevent excess hemodilution due to additional cardioplegic solution ( Taguchi et al ., 2014 ; Mihara et al ., 2017 ; Yokoyama et al ., 2017 ).…”

“…Therefore, although the time proportion of surface-cooling and blood rewarming with sHT and CPB was about 2:1 ( Fig. 2 ) ( Yokoyama et al ., 2017 ), the time proportion of the rates of blood cooling and rewarming was reversed to about 1:2 with the bHT and CPB ( Fig. 3 ) ( Mihara et al ., 2017 ).…”

“…, the procedures mostly used in persons are mitral valve repair, valve replacement with a mechanical valve or bioprosthetic valve 33,35,45 . There are very few comparative studies of veterinary patients that have undergone valve surgery, so there is not a clear criteria for which techniques have the best outcome 45,46,47 . Recent reports reveal that the surgical option of mitral valve repair has a better outcome in dogs, using the mitral chordal replacement, with mitral annuloplasty 33,45,46 , valvuloplasty 47 or resection of the prolapsed segment 8 .…”
