Treatment of tumour recurrence after resection of hepatocellular carcinoma. Analysis of 97 consecutive patients

Abstract: Objective: To evaluate the long-term results of aggressive treatment of HCC recurrence. Methods: Two hundred and nine consecutive patients underwent hepatic resection for HCC in our hospital. Tumour recurrence was diagnosed in 97 (51%) of the 190 patients with curative resection. Sixteen underwent hepatic resection: two right hepatectomies, one threesegmentectomy, one left hepatectomy, five two-segmentectomies, six segmental resections and one subsegmentectomy. Two patients with metastasis in the spine were su… Show more

“…However, two patients presented recurrence near the hepatic stump, although the margin exceeded 5 mm in both. This frequency is slightly higher than that obtained in our study of open liver resection for HCC [31]. The exact location of the tumor and transection planes must be precisely defined to ensure that an adequate surgical margin is obtained.…”

“…Patients were followed up with physical examination, ultrasonography or CT, and serum AFP levels every 3 months for the first year and every 3-6 months thereafter. Suspected intrahepatic recurrence was confirmed by using a CT scan, hepatic angiography, elevated AFP level and/or percutaneous liver biopsy [19]. Determination of treatment modalities for recurrent HCCs was based on liver function, tumor size, number and site of the tumors and any concurrent extrahepatic recurrence.…”

“…Although surgical resection is considered to be the most effective therapeutic method for treatment of patients with primary hepatic carcinoma, the high recurrence rate and early distant metastasis of HCC after surgery remains frustrating (Kobayashi et al, 2006;Tralhao et al, 2007). Identification of factors involved in oncogenesis of HCC may facilitate improvement of early diagnosis and therapeutic approaches (Ji et al, 2014).…”
