Treatment of two postoperative endophthalmitis cases due to Aspergillus flavus and Scopulariopsis spp. with local and systemic antifungal therapy

Abstract: Background: Endophthalmitis is the inflammatory response to invasion of the eye with bacteria or fungi. The incidence of endophthalmitis after cataract surgery varies between 0.072-0.13 percent. Treatment of endophthalmitis with fungal etiology is difficult.

“…Caspofungin eye drops are effective for treatment of fungal keratitis in rabbits (11, 18) and have good penetration into animal eyes with inflammation or previous corneal abrasion (10, 23). Clinically, caspofungin is administered mostly intravenously (1,4,6,9,19,20) or intravitreally (24) for ocular mycosis. The successful use of topical caspofungin alone (13) or in combination with voriconazole (22) has been reported for two patients.…”

“…They concluded that due to a lack of clinical correlation with in vitro susceptibility, successful treatment requires surgical debridement [15]. Aydin et al, on the other hand, described 2 patients with postoperative endophthalmitis caused by Scopolariopsis that were successfully treated with local and systemic antifungal therapy [17]. Like most of the patients with sinonasal Scopulariopsis our patient was also treated with both antifungals and wide local excision of the infected tissue with favorite outcome.…”

“…Invasive infections with the fungus have been described in immunocompromized patients in multiple organs like heart, brain, lung and cornea [14][15][16][17]. In recent years there has been an increase in the number of publications on the invasive form of the disease with various treatment modalities, none of them proven superior.…”
