Treatment of Ulcerative Colitis with Camostat Mesilate, A Serine Protease Inhibitor.

Abstract: Wewere able to induce and maintain remission with camostat mesilate, a serine protease inhibitor, in two patients with ulcerative colitis, to whomsalicylazosulfapyridine could not be administered due to previous side effects. The enzymatic activity of proteases from granulocyte, pancreatic juice and bacteria is possibly harmful to the inflammed colonic mucosa. Camostat mesilate can be expected to have an anti-inflammatory effect on the damagedmucosaof inflammatory bowel disease.

“…The addition of EGF at a dose of 5 Ìg/ml to the system (P) caused a similar increase in restitution as that seen when PSTI was added at a dose of 10 Ìg/ml (o). in the proteolytic/anti-proteolytic load on the mucus layer is probably important in explaining the increased turnover of mucus in these patients; luminal serine proteases are increased (probably of bacterial origin) [25] and colonic mucosal PSTI levels are concomitantly reduced [26]. Importantly, the reduction in mucosal PSTI levels remains even in quiescent colitis.…”

“…inhibitor, induced long-term remission in two patients with refractory UC. Camostat mesilate could act by inhibiting specific inflammatory proteases, thereby decreasing tissue damage 41 . The serum transaminase level is most widely used as a measure of hepatic injury, due to its ease measurement and high degree of sensitivity.…”

“…There is a report of another protease inhibitor, camostat mesilate, that was used to treat two patients with UC [3]. While the patients did experience some temporary relief of symptoms, the study was not blinded or placebo-controlled, and the use of camostat mesilate in UC patients has not been further investigated.…”

“…The prevalence of UC is highest in the US and Europe and in developed countries generally. Epidemiological studies indicate that a diet high in soybeans and other legumes may contribute to an attenuated incidence of UC [2,3].…”
