Tricenter analysis of cofactors and comorbidity in patients with pyoderma gangrenosum

Abstract: The present data analysis confirms the association of PG with metabolic syndrome and neoplasms. In the future, these aspects should be included in the targeted diagnostic workup of patients with PG and subsequently treated in a timely fashion.

“…6 Recently, it has been hypothesized that PG is a systemic immune-mediated inflammatory disease rather than a purely skin disease. 7 In fact, the physiopathology of both PG and associated cardiovascular diseases implies an increase in the inflammatory response and in the production of some interleukines, particularly through T-helper 17 cells. 8 In this context, cardiovascular diseases should be potential comorbidities of PG, and should be ruled out in patients with PG, especially when corticosteroid treatment is needed.…”

“…Our findings of elevated CRP levels and neutrophilia in PG indicates the presence of systemic inflammation, which is increasingly suggested by the field . Additionally, the reduced likelihood of pathogenic bacteria in PG lesions could be explained by the exacerbated neutrophil response.…”

“…Our findings of elevated CRP levels and neutrophilia in PG indicates the presence of systemic inflammation, which is increasingly suggested by the field. 8 Additionally, the reduced likelihood of pathogenic bacteria in PG lesions could be explained by the exacerbated neutrophil response. Cardiovascular comorbidities like arterial hypertension, peripheral artery occlusive disease, metabolic syndrome and diabetes mellitus appear significantly more often in HYTILU, which has also been reported previously by our group, compared with PG.…”

“…The mean leucocyte and neutrophil counts were 10.5 × 10 9 /L and 8.36 × 10 9 /L, respectively. This is the first study reporting evidence of elevated CRP, neutrophilia, and leukocytosis in PG patients, which is indicative of systemic inflammation in PG patients, as is more and more suggested in the field [35] . However, potential bias with regard to these data is that, retrospectively, a causative or coexistent infection could not be fully ruled out.…”
