Trigeminal herpes zoster: early recognition and treatment are crucial

Abstract: Reactivation of varicella zoster virus (VZV) is not uncommon in older patients, particularly in cases of chronic autoimmune disorders and in patients taking immunosuppressant drugs. We present a case of a 57-year-old woman presenting with severe herpes zoster infection, involving the maxillary and ophthalmic branches of the trigeminal nerve. Despite an initial delay in instigating crucial antiviral treatment, the patient achieved an excellent recovery, with only some mild scarring at 2 months postinfection. Tr… Show more

“… 8 , 9 Besides PHN, various other complications such as scarring and disfigurement, encephalitis, and even permanent loss of vision can also occur. 10 Therefore, early diagnosis of lesions and the prompt use of antivirals can help manage the orofacial herpes zoster infection in dental practice.…”

“…Trigeminal postherpetic neuralgia (TPHN) is a special condition of PHN that is caused by the invasion of VZV in the semilunar ganglion or its branches within the trigeminal nerve. Although it accounts for only 5% of cases of PHN, it remains notoriously resistant to treatment (4)(5)(6). Common treatment approaches for TPHN include medication, nerve blocks, destructive procedures, pulsed radiofrequency, and radiofrequency thermoablation (5,(7)(8)(9)(10).…”

Common treatment approaches for TPHN include medication, nerve blocks, destructive procedures, pulsed radiofrequency, and radiofrequency thermoablation (5,(7)(8)(9)(10). However, these methods have demonstrated limited effectiveness, particularly in patients with chronic disease.…”

“…Some of the reported medical complications following herpes zoster include post-herpetic pain, vasculopathy or vasculitis, neurological complications, meningitis/meningoencephalitis, cerebellitis, myelopathy and ocular complications including retinal necrosis (27,28). Those affected by such post-herpetic complications may suffer prolonged or life-long effects of herpes zoster (26,29,30).…”
