Nonalcoholic liver disease (NAFLD) is manifested with a wide spectrum of clinical symptoms and is closely associated with the metabolic syndrome, inflammation, and mitochondrial dysfunction. Although the mechanism of mitochondrial dysfunction in NAFLD is still not fully elucidated, multiple studies have demonstrated evidence of molecular, biochemical, and biophysical mitochondrial abnormalities in NAFLD. Given the association between NAFLD and mitochondrial dysfunction, the aim of this study is to analyze circulating levels of Krebs cycle intermediates in a cohort of NAFLD-affected individuals and matching healthy controls and to correlate our findings with the liver function metrics. Standard serum biochemistry and Krebs cycle intermediates were analyzed in NAFLD (n = 22) and matched control (n = 67) cohorts. Circulating levels of isocitrate and citrate were significantly (p < 0.05) elevated in the NAFLD cohort of patients. The area under the curve (AUROC) for these two metabolites exhibited a moderate clinical utility. Correlations between plasma Krebs cycle intermediates and standard clinical plasma metrics were explored by Pearson’s correlation coefficient. The data obtained for plasma Krebs cycle intermediates suggest pathophysiological insights that link mitochondrial dysfunction with NAFLD. Our findings reveal that plasma isocitrate and citrate can discriminate between normal and NAFLD cohorts and can be utilized as noninvasive markers of mitochondrial dysfunction in NAFLD. Future studies with large populations at different NAFLD stages are warranted.

Background Metabolic-associated fatty liver disease (MAFLD), also known as the hepatic manifestation of metabolic disorders, has become one of the most common chronic liver diseases worldwide. The associations between some oral resident microbes and MAFLD have been described. However, changes to the oral microbial community in patients with MAFLD remain unknown. Methods In this study, variations to the supragingival microbiota of MAFLD patients were identified. The microbial genetic profile of supragingival plaque samples from 24 MAFLD patients and 22 healthy participants were analyzed by 16S rDNA sequencing and bioinformatics analysis. Clinical variables, including indicators of insulin resistance, obesity, blood lipids, and hepatocellular damage, were evaluated with laboratory tests and physical examinations. Results The results showed that the diversity of the supragingival microbiota in MAFLD patients was significantly higher than that in healthy individuals. Weighted UniFrac principal coordinates analysis and partial least squares discriminant analysis showed that the samples from the MAFLD and control groups formed separate clusters (Adonis, P = 0.0120). There were 27 taxa with differential distributions (linear discriminant analysis, LDA༞2.0) between two groups, among which Actinomyces and Prevotella 2 spp. were over-represented in the MAFLD group with highest LDA score, while Neisseria and Bergeyella spp. were more abundant in the control group. Co-occurrence networks of the top 50 abundant genera in the two groups suggested that the inter-genera relationships were also altered in the supragingival plaque of MAFLD patients. In addition, as risk factors for the development of MAFLD, insulin resistance was positively correlated with the abundances of Granulicatella, Veillonella, Streptococcus, and Scardovia spp., while obesity was positively correlated to the abundances of Streptococcus, Oslenella, Scardovia, and Selenomonas spp. Metagenomic predictions based on Phylogenetic Investigation of Communities by Reconstruction of Unobserved States revealed that pathways related to sugar (mainly free sugar) metabolism were enriched in the supragingival plaque of the MAFLD group. Conclusions In conclusion, as compared to healthy individuals, component and interactional dysbioses were observed in the supragingival microbiota of the MAFLD group, which could be further explored as potential oral biomarkers of the MAFLD.

Background: Non-alcoholic fatty liver disease(NAFLD) has a high prevalence in the general population worldwide. Both triglycerides (TG) and total cholesterol (TC) are correlated with the prevalence of NAFLD. The study purpose is to determine whether TG/TC is an effective method to screen NAFLD in the general Chinese population.Methods: 93,449 subjects with average age of 43.7 ± 14.3 years were included in this cross-sectional study. Multiple logistic regressions and receiver operator characteristic curve (ROC) analyses were performed.Results: Among these subjects, 16,138 (42.7%) men and 9,591 (17.2%) women were diagnosed with NAFLD. Subjects in the higher quartiles of TG/TC had a higher prevalence of NAFLD. After adjusting multiple confounding factors, the odds ratio (OR) for NAFLD in the highest compared with the lowest quartile was 4.08 (95%CI 3.64, 4.57) in men and 4.65 (95%CI 4.14, 5.21) in women. Moreover, ROC analyses suggested TG/TC showed high diagnostic ability for detecting NAFLD, and the areas under the curves (AUC) in men and women were 0.920 (95%CI 0.917, 0.923) and 0.863 (95%CI 0.859, 0.867), respectively. Furthermore, the diagnostic ability was significantly higher in younger age groups. The AUC in 18–34 and 35–44 years group were 0.943 (95%CI 0.939, 0.946) and 0.921 (95%CI 0.917, 0.925), respectively. Besides, the women and the young people had a greater negative predictive values (90.84% for women, 93.13% for aged 18–34).Conclusion: TG/TC shows a high diagnostic accuracy for identifying NAFLD in young women, providing an important clue to establish a new tool to screen NAFLD particularly in Chinese young females.
