Recombinant tumor necrosis factor (TNF)-related apoptosis-inducing ligand (TRAIL), as a novel cancer therapeutic, is being tested in phase II and III clinical trials; however, TRAIL resistance remains a big obstacle preventing its clinical application. Considering that TRAIL-induced apoptosis through death receptors DR4 and DR5, their activation may be an alternative pathway to suppress TRAIL resistance. In this study, a negative correlation between DR5 expression and TRAIL resistance was observed, and miR-133a was predicted to be the most promising candidate to suppress DR5 expression. Further investigation demonstrated that miR-133a knockdown dramatically suppressed TRAIL resistance in glioblastoma in vitro and in vivo. An NF-κB family member, phosphorylated IκBα (P-IκBα), was shown to be stimulated by miR-133a, leading to the activation of this signaling. Finally, miR-133a was found to be inversely correlated with DR5 expression in human clinical specimens. In conclusion, our data demonstrate that miR-133a promotes TRAIL resistance in glioblastoma by suppressing DR5 expression and activating NF-κB signaling.

The Incidence of Anterior Pituitary Hormone Deficiencies in Patients with Pituitary Microadenoma and Idiopathic Hyperprolactinaemia. A Retrospective Single Centre Study

Introduction: Patients with microprolactinoma and idiopathic hyperprolactinaemia are not generally considered to be at risk of hypopituitarism and are therefore not routinely screened for this abnormality. Aims: We aimed to establish the frequency and clinical significance of anterior pituitary hormone deficiencies, comparing patients with radiologically proven microprolactinomas and idiopathic hyperprolactinaemia. Study Design: We retrospectively examined the case notes of 242 patients with hyperprolactinaemia from our centre. Patients who did not fit the profile of surgically naïve microprolactinoma or idiopathic hyperprolactinaemia or who had incomplete data were excluded, resulting in a study group of 185 patients. Results: Out of 242 patients, 185 patients were identified with microprolactinoma and idiopathic hyperprolactinaemia. 47 (20 %) were male and 148 (80 % ) were female with mean age 35.4 ± 13.7. 87(47%). Four types of hypofunctioning pituitary gland were seen such as panhypopituitarism, secondray hypogonadism, growth hormone deficiency and central hypothroidism and were associated with more frequent normal MRI. Patients with MRI evidence of microprolactinoma were identified, three (3.4%) of whom had one or more anterior pituitary hormone deficiencies. A total of 98 (53%) patients with MRI-negative idiopathic hyperprolactinaemia were identified, twelve (12.2%) of whom had one or more anterior pituitary hormone deficiencies. Patients in the MRI-positive and MRI-negative groups had panhypopituitarism, hypogonadotrophic hypogonadism and growth hormone deficiency that required hormone. Conclusion: The current study shows an increased frequency anterior pituitary hormone deficiency in patients with idiopathic hyperprolactinaemia, not with pitutary microadenoma. A prospective study would be required to assess the underlying cause for these abnormalities, as they suggest a nontumour pan-pituitary process. Limitations: Question of clustering of cases within the study region and limited study sample size.

RESUMO -No presente estudo, foram pesquisados os tumores do sistema nervoso central (SNC) diagnosticados na cidade de Curitiba entre 1990 e 1994. Realizaram-se 2371 biópsias de SNC das quais 1340 casos eram neoplasias primárias. Dentre as neoplasias primárias de SNC 304 (22,6%) eram meningiomas. Duzentos e nove meningiomas ocorreram em mulheres (68,7%) e 95 ocorreram em homens (31,3%). As idades variaram de 3 a 90 anos, com média de 45,8 anos. Duzentos e oitenta meningiomas localizavam-se no cérebro, 10 na medula espinhal, 9 no cerebelo e 5 na emergência de nervos cranianos. Na classificação histológica verificou-se que 294 (96,7%) eram do tipo clássico, 6 malignos (ou anaplásicos), 3 atípicos e 1 papilar. No grupo dos meningiomas clássicos 267 eram meningoteliais, 10 psamomatosos, 5 fibroblásticos, 5 microcísticos, 5 transicionais e 2 angiomatoses. Os autores concluem que as neoplasias meníngeas representaram o grupo mais frequente de tumores primários do SNC e apresentaram características epidemiológicas e anátomo-patológicas similares às da literatura mundial, na qual é dada ênfase à dificuldade de estabelecer critérios biológicos e histológicos de malignidade neste grupo de tumores. Meningiomas: epidemiological and pathological studies of 304 casesABSTRACT -The authors have retrospectively reviewed all tumors of central nervous system (CNS) operated at the most important neurosurgery hospitals of Curitiba in a 5-year period (1990)(1991)(1992)(1993)(1994) and found 304 (22.4%) cases of meningioma. Age mean of the patients was 48.5 years, with a range of 3 to 90 years. A marked female preponderance (68.7%) was noted. The most common tumor location was brain (n=280) and the remaining cases occurred in spinal cord (n=10), cerebellum (n=9) and cranial nerves (n=5). Histologically, there were 294 (96.7%) meningiomas of the classical type, six malignant or anaplastic, three atypical and one papillary. Two hundred and sixty seven classical meningiomas were from the meningotelial subtype, ten psamomatousos, five fibroblastic, five microcystic, five transicional and two angiomatous.
