Twenty-Four-Hour Ambulatory Blood Pressure Variability Associated With Cerebral Small Vessel Disease MRI Burden and Its Progression in Inpatients With Cerebrovascular Disease

Abstract: Background: Lacunar infarcts, white matter lesions, cerebral microbleed, enlarged perivascular space and brain atrophy are regarded as magnetic resonance imaging (MRI) manifestations of cerebral small vessel disease (cSVD). 24-hour blood pressure variability (BPV) has been reported to relate with cerebral small vessel disease, but the impact of 24-h BPV on the total MRI cSVD burden and its progression in inpatients with cerebrovascular disease has not been investigated yet. Methods: We enrolled inpatients with… Show more

“…Several studies have previously reported that CSVD marker numbers or total load of CSVD impacts multiple cognitive domains and increases the risk of dementia and cognitive decline. [31][32][33][34] Most of these studies were in older or specific populations (eg, stroke, hypertension people et al). Before adjustment, we did find 3 image markers and 4 marker group associated with NCT-A (DST) variables, after age adjustment, only 4 marker group remained statistically significant.…”

“…Last, defining progression as binary endpoint instead of on a continuous scale may also have reduced power to detect an association. Previous studies on BPV and WMH progression in ischemic stroke patients have shown conflicting results [13] , [14] , [15] . Studies that did find an association assessed short-term BPV and applied home-based instead of office-based BP measurements [ 14 , 15 ], suggesting that timing and method of BP measurement matters.…”

“…Rianne et al provided the clinical evidence that day-to-day systolic BPV was an independent risk factors for cSVD burden except diastolic BPV or mean BP in participants from memory clinic [12]. Fan et al demonstrated that greater SBP wSD and DBP wSD of patients with cerebrovascular diseases related to the total cSVD burden progressed [17].…”
