Two-Layer Tissue Engineered Urethra Using Oral Epithelial and Muscle Derived Cells

Abstract: Two-layer tissue engineered urethras were engineered using cells harvested by minimally invasive oral punch biopsy. Results suggest that this technique can encourage regeneration of a functional urethra.

“…Previously, Mikami et al . 38 fabricated two-layer tissue-engineered urethra grafts by combining autologous oral epithelial cell sheets with collagen mesh matrices seeded with muscle cells that were grafted to cover a 2-cm urethral defect in a canine model and promoted the regeneration of a functional urethra. However, the 2-layer urethra without a fibrous layer is a loose construction that may lack adequate mechanical strength and revascularization for the submucosal layer of urethral vessels could not be regenerated.…”

“…It is showing promising results in preclinical and clinical trials for future implication as an alternative cell source for urethral tissue engineering 35,106,107. In a recent clinical study involving 12 patients, stricture recurrence in the anterior urethra occurred in two patients only 108.…”

“…However, the process of graft take in an injured urethra may substantially differ from that in a normal urethral bed. What is more, only four (Mikami et al , ; Orabi et al , ; Xie et al , , ) of 28 preclinical studies used large animals (dogs), and only two of them used male dogs (Mikami et al , ; Orabi et al , ). While we are aware that, anatomically, the female and male urethras are different, and that spongiofibrosis, being the major cause of strictures in males, never occurs in females, we still decided to include the studies conducted on female animals in our review.…”

“…Mikami et al () formed tubularized tissue‐engineered urethras comprised of collagen mesh matrices and autologous cells of two types – keratinocytes and buccal mucosa smooth muscle cells. These grafts were used to repair urethral defects in the experimental group (10 dogs) and were not applied to defects in the control group (10 dogs).…”
