Two-staged hybrid treatment of persistent atrial fibrillation: short-term single-centre results

Abstract: The sequential, two-staged hybrid strategy (surgical thoracoscopic followed by catheter ablation) is feasible and safe with a high post-procedural success and seems to represent the optimal treatment with low risk load and potentially long-term benefit for patients with a persistent and long-standing persistent form atrial fibrillation.

“…The studies of Bulava et al [27] and Kurfirst et al [28] are considered overlapping; of these two, Bulava et al comprises the most recent study and encloses the biggest population. The same accounts for two articles written by La Meir et al [29, 30], one by Kumar et al [31], and one by Pison et al [32], compared to a larger study by Pison et al [15].…”

“…It is also possible to ablate arrhythmia triggers and inducible atrial tachycardias after the surgical ablation; these would otherwise be left untreated. Studies published to date included simultaneous or sequential hybrid procedures with reported success rates from 83–94 %, but the experience has only been with bipolar or monopolar radiofrequency energy [1215]. The only report on hybrid ablations with cryoenergy used during the surgical part of the procedure revealed an overall freedom from atrial tachyarrhythmias to be 86 % at 12 months.…”

“…Minimally invasive approaches have also been utilized in conjunction with treatment for AF, with bilateral video-assisted thoracoscopic pulmonary vein isolation, atrial ganglia ablation, ligament of Marshall division and LAA staple excision, with a reported success rate of 50 -90% in maintaining sinus rhythm for paroxysmal AF, 30 -85% for persistent AF and 30 -75% for long-standing persistent AF, [117][118][119] and up to 90% if followed by a "touch-up" catheter ablation procedure. 120 The effects on stroke risk reduction of these hybrid approaches is currently unknown.…”
