The sensitivity between double contrast barium enema (DCBE) and sigmoideoscopy in diagnosing neoplastic lesions in the sigmoid colon was compared in patients with diverticulosis. In 52 patients with severe diverticulosis (2: 15 diverticu1as) the DCBE detected one out of 4 polyps found by sigmoideoscopy. In the remaining 54 patients with mild diverticulosis « 15 diverticulas) DCBE detected 7 out of 10 polyps found by sigmoideoscopy. Successful bowel preparation did not influence the outcome of the DCBE. Sigmoideoscopy was incomplete in 17 (16%) of the patients; females were more difficult to examine than males (p=0.012), as were those with a previous pelvic operation (p = 0.032). We conclude that neither DCBE nor sigmoideoscopy alone is sufficient to detect all neoplastic lesions in the sigmoid colon in patients with sigmoid diverticulosis of the colon.

A total of 2,118 consecutive patients, examined with double-contrast examination of the large bowel (DCE), were evaluated radiographically, clinically, and endoscopically for up to 48 months. A total of 402 polyps were diagnosed during the follow-up period. Of these, 307 were revealed at the first DCE and 95 at subsequent endoscopy or autopsy. The accuracy of the DCE for demonstrating or excluding patients with polypoid lesions of the colon was 97%, whereas the predictive value of a positive DCE report was 96% and a negative one, 97%. However, the ability to demonstrate every known polyp at DCE was only 63%, although the probability of a polypoid lesion demonstrated at DCE being a true polyp was 92%.

During a 9-month period, 2 590 patients were referred for radiographic examination of the large bowel. All but 55 of these examinations were performed according to the double contrast examination method. The patients were followed prospectively from 40 to 48 months. Carcinoma of the colon or rectum was disclosed in 96 patients and other malignant lesions in 7 others. The carcinomas in 93 patients were diagnosed at the primary DCE. During the follow-up period, carcinoma of the colon was revealed in another 10 patients, the malignancy having been obfuscated by diverticulitis in 3 patients and considered polyps in 6 others. The accuracy of the double contrast method was calculated at 99.3 per cent.
