Introduction In the world, the second most common cause of mortality is cancer, and its prevalence is increasing in both developing and developed states. Cancer therapy has severe side effects for people with cancer. The selection of natural ingredients in the form of herbal plants is expected to provide therapeutic effectiveness with low side effects. A total of 6000 plant species are utilized as herbal medicines in Indonesia by the local population for various ailments. Objective To describe the potential of Indonesian herbal plant products as a cancer therapy in vitro. Methods This systematic review is based on Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA-SR) guidelines. The selection of articles published in the last 5 years (2018–2023) from Pubmed, Ebsco, Garuda portal, ScienceDirect, Cochrane, and Webscience was carried out in January 2023 with the keyword “Indonesian herbal” AND/OR “Anti-cancer” AND/OR “In Vitro”. Risk of bias assessment using OHAT tools. Results A total of 1,816,511 articles then through inclusion, exclusion criteria screening, and the risk of bias were obtained 23 (twenty-three) low risk of bias articles. Some herbal plant products such as Soursop ( Annona muricata L .), Nyamplung ( Calophyllum spp .), Benalu Cengkeh/Clove Benalu ( Dendrophthoe pentandra ), Rumput Mutiara/Pearl Grass ( Hedyotis corymbosa L .), Rasamala ( Altingia excelsa ), Sarang Semut/Anthill plant ( Myrmecodia pendans ), Basil ( Ocimum sanctum Linn .), and Tepus ( Zingiber griffithii ) showed different potentials for activity as an in vitro anti-cancer therapy. The three Indonesian herbal plants that are most studied in vitro as anti-cancer are Soursop, Rasamala, and Benalu Cengkeh/Clove Benalu. Conclusion The most widely studied Indonesian herbal plant in vitro as an anti-cancer is Soursop, while the anti-cancer activity that is widely reported is by inhibiting cell proliferation through intrinsic pathways.

Latar Belakang: Angka kematian kanker serviks di Indonesia sangat tinggi. Kebanyakan pasien datang pada stadium lanjut karena pada tahap awal tidak menimbulkan gejala yang berarti. Kandungan pada daun sirsak berpotensi menghambat viabilitas sel kanker. Objektif: Penelitian ini memiliki tujuan mengetahui apakah ekstrak daun sirsak mampu menghambat 50% viabilitas HeLa cell line. Metode: Penelitian eksperimental in vitro ini menggunakan HeLa cell line yang diinkubasi selama 24 jam yang telah diberikan perlakuan ekstrak daun sirsak sesuai dengan kelompok konsentrasi yang telah ditentukan. Uji aktivitas sitotoksik ekstrak daun sirsak dengan etanol 95% sebagai pelarut dilakukan dengan metode MTT (microtetrazolium). Pembacaan hasil dilakukan dengan menggunakan xMark microplate reader. Persentase viabilitas dapat dihubungkan dengan log konsentrasi dengan menggunakan rumus regresi. Hasil: Hasil penelitian menunjukkan makin tinggi konsentrasi makin rendah viabilitas sel. Konsentrasi penghambatan 50% viabilitas HeLa cell line dengan masa inkubasi selama 24 jam adalah 112,2 μg/ml. Kesimpulan: Perbedaan konsentrasi ekstrak daun sirsak dapat berpengaruh terhadap viabilitas sel kanker serviks HeLa dengan menghasilkan persentase viabilitas yang berbeda. Nilai penghambatan 50% viabilitas sel HeLa menunjukkan potensi ekstrak daun srisak sebagai antikanker.

Breast cancer has a significant morbidity and death rate worldwide, thus various therapeutic strategies, including the phytopharmaca approach, must be researched further. Bitter melon (Momordica charantia) extract is predicted to have anticancer properties due to its influence on Caspase-9 and BcL2 proteins, which will be shown in this in vitro investigation utilizing MCF-7 cell culture. This study was an in vitro laboratory experimental study with a post-test-only control group design. The doses of Momordica charantia extract used were 125 μg/ml, 500 μg/ml, and 1,000 μg/ml. Caspase-9 and BcL2 protein expression in MCF-7cell culture was measured by immunocytochemistry method, Shapiro Wilk, Levene Test, One Way Anova, and Post Hoc LSD. Administration of Momordica charantia extract at a dose of 500 μg/ml and 1,000 μg/ml significantly increased the average expression of Caspase-9 protein (p = 0.044 and p = 0.004 respectively), but not at a dose of 125 μg/mL (p = 0.125). Administration of bitter melon extract at doses of 125 μg/ml, 500 μg/ml, and 1,000 μg/ml significantly reduced the average BcL2 protein expression (p < 0.001, p < 0.001, and p = 0.001 respectively). Momordica charantia extract has the potential to be employed as an anti-breast cancer agent due to its ability to increase the average expression of Caspase-9 protein while reducing the average expression of BcL2 protein.
