SummaryThis article reviews recently published studies investigating the MRI and sonographic diagnosis of masticatory muscle myalgia in temporomandibular disorder patients. The MRI and sonographic features of muscle after treatment are also discussed. Literature published within the last 15 years was obtained from the PubMed database using the following Mesh terms: magnetic resonance imaging (MRI) or sonography, masticatory muscle pain, and treatment. MRI and sonography enable accurate visualization and evaluation of the masticatory muscles, thereby increasing our understanding of pathology and cause of pain associated with these muscles. Although therapeutic efficacy is often evaluated based on clinical findings, MR and sonographic imaging studies may also be valuable.

Ö ÖZ ZE ET T Toplumda yüksek oranda gözlemlenen temporomandibuler eklem (TME) rahatsızlıkları çene, boyun ve baş ağrıları, eklem sesleri gibi pek çok semptomla bireylerin yaşam kalitesini düşürmek-tedir. Bu rahatsızlıklar tedavi edilmediği takdirde ilerlemekte ve TME'de kalıcı hasara sebep olabilmektedir. TME rahatsızlıkları için pek çok tedavi yöntemi bildirilmiştir. Bu tedavi yöntemleri geri dönüşümlü (noninvaziv) ve geri dönüşümsüz (invaziv) tedaviler olmak üzere ikiye ayrılmıştır. Geri dönüşümlü tedaviler arasında bilgilendirme yöntemi, fizik tedavi yöntemleri, farmakolojik tedavi, psikiyatrik destek tedavisi ve oklüzal splint tedavileri yer alır iken; geri dönüşümsüz tedaviler oklüzal uyumlama ve cerrahi girişimleri içermektedir. TME rahatsızlıklarında geri dönüşümsüz tedavilerden önce daha konservatif yöntemler olan geri dönüşümlü tedaviler denenmesi temel prensiptir. Tavsiye edilen tedavi yöntemlerinin etkinliklerini araştırmak amacıyla pek çok çalışma yapılmıştır. Bununla birlikte, tedavi etmede ya da semptomları azaltmada hangi tedavi yönteminin ya da hangi splint tipinin daha etkin olduğuna dair kesin bir kanıt mevcut değildir. Etki mekanizmaları ve tedavi başarıları tartışmalı olmasına rağmen, TME rahatsızlıkları tedavisinde en çok kullanılan yöntem oklüzal splint tedavisidir. Diş hekimliğinde her biri farklı özel durumlar için olmak üzere birçok oklüzal splint kullanılmaktadır. Splint endikasyonları TME inflamasyonu, eklem içi dü-zensizlik, cerrahi sonrası tedavi, hipertonik yüz kasları, diş sıkma ve gıcırdatma olarak belirtilmektedir. These disorders progress when they are not treated and may cause permanent damage to the TMJ. Many treatment modalities have been reported for TMJ disorders. These treatment methods are divided into two as non-invasive treatments and invasive treatments. Non-invasive treatments include informative methods, physical therapy, pharmacological treatment, psychiatric support therapy and occlusal splint therapies and invasive treatments include occlusal adjustment and surgical interventions. Non-invasive treatments, which are more conservative methods should be employed before invasive treatments for TMJ disorders. Many studies have been conducted to investigate the effectiveness of recommended treatment modalities. Ultrasonographic Assessment of Masseter and Anterior Temporal Muscle Thickness and Internal Structure in Young Adult Patients With Bruxism

ObjectiveThe current study is a comparative cross‐sectional analysis study to determine the thickness and internal structure of the masseter muscle (MM) and anterior temporal muscle (ATM) in patients with bruxism by ultrasonography (USG).Materials and MethodsA total of 100 patients (36 males, 64 females), 50 with bruxism and 50 without bruxism, aged 20–30 years were included in the study. All patients were investigated with MM and ATM USG. The thickness of the muscles was measured at rest and during clenching and the internal structure at rest was classified as Type I, II, and III. Differences in the internal structure between bruxism and non‐bruxism groups were determined using the Chi‐square test (p < 0.05).ResultsThe most common internal structure in bruxism was Type 2 in MM (74%) and Type 1 in ATM (46%), whereas in non‐bruxism, Type 1 in MM (58%) and Type 1 in ATM (80%) were found most frequently. While there was a statistically significant difference in the internal echogenic pattern in the right MM and ATM in bruxism (p < 0.05), no significant difference was observed on the left side (p > 0.05). Bruxism patients had higher rest and clench thicknesses than non‐bruxism patients on both sides in MM and ATM, however, this did not result in a statistically significant difference (p > 0.05).ConclusionsThe results obtained showed that the evaluation of internal structure and thickness differences via USG is crucial in understanding the nature of the bruxism process affecting the masseter and ATM, facilitating its clinical diagnosis and leading to its treatment.
