Ultrasound-Guided Placement of Gold Fiducial Markers for Stereotactic Partial-Breast Irradiation

Abstract: All 51 postoperative patients underwent successful fiducial placement without complications. Our technique of placing gold fiducial markers in proximity to the seroma cavity is considered safe and effective for breast cancer patients being treated with APBI using robotic-based SBRT.

“…Prior to the treatment, three to four gold fiducials were placed near the surgical bed with ultrasound guidance by a certified radiologist together with the treating radiation oncologist, in local anaesthesia. For optimal tracking, the maximum and minimum distances between fiducial markers were less than 10 cm and more than 2 cm, and the markers were placed at least 1-2 cm away from the seroma cavity, as recommended by Seiler et al [38]. One week after gold marker placement, a new simulation CT was performed in a supine position with both arms next to the body with 1.25 mm axial slice spacing.…”

“…Forty-three patients were randomly selected from the cohort enrolled at our institutional clinical trial, which is a single-arm, prospective 5-fraction dose escalation stereotactic radiotherapy study conducted on Cyberknife system. For each enrolled patient, four to five gold fiducial markers (CIVCO Medical Solutions, Orange City, IA) were systematically implanted at least 2-cm apart at the edge of the lumpectomy cavity [ 15 ]. Each gold fiducial marker was 3-mm in length and 1.2-mm in diameter.…”

“…Three to four of these markers were placed internally before simulation at surgery or under ultrasound guidance by a radiologist in conjunction with the treating radiation oncologist, around the lumpectomy cavity. We have previously published an article providing details of ultrasound-guided fiducial placement in the breast (12). Daily localization and fiducial tracking was achieved using the Synchrony Respiratory Tracking System associated with the Cyberknife radiosurgery system per established system standards (Cyberknife, Accuray, Sunnyvale, CA).…”
