BackgroundAtrial fibrillation (AF) is a common form of arrhythmia that is associated with increased risk of stroke and mortality. Detecting AF before the first complication occurs is a recognized priority. No previous studies have examined the feasibility of undertaking AF screening using a telehealth surveillance system with an embedded cloud-computing algorithm; we address this issue in this study.ObjectiveThe objective of this study was to evaluate the feasibility of AF screening in nonmetropolitan areas using a telehealth surveillance system with an embedded cloud-computing algorithm.MethodsWe conducted a prospective AF screening study in a nonmetropolitan area using a single-lead electrocardiogram (ECG) recorder. All ECG measurements were reviewed on the telehealth surveillance system and interpreted by the cloud-computing algorithm and a cardiologist. The process of AF screening was evaluated with a satisfaction questionnaire.ResultsBetween March 11, 2016 and August 31, 2016, 967 ECGs were recorded from 922 residents in nonmetropolitan areas. A total of 22 (2.4%, 22/922) residents with AF were identified by the physician’s ECG interpretation, and only 0.2% (2/967) of ECGs contained significant artifacts. The novel cloud-computing algorithm for AF detection had a sensitivity of 95.5% (95% CI 77.2%-99.9%) and specificity of 97.7% (95% CI 96.5%-98.5%). The overall satisfaction score for the process of AF screening was 92.1%.ConclusionsAF screening in nonmetropolitan areas using a telehealth surveillance system with an embedded cloud-computing algorithm is feasible.

Objetivos: evaluar la efectividad del cribado de pulso irregular por el farmacéutico comunitario mediante el pilotaje de una metodología y procedimiento para una campaña de detección de pulso irregular en las farmacias comunitarias españolas. Métodos: estudio descriptivo transversal realizado por farmacéuticos comunitarios socios de SEFAC entre los días 24 a 29 de noviembre de 2016 y los días 5 a 11 de junio de 2017. Criterios de inclusión: usuarios ≥40 años, sin FA o con FA sin tratamiento anticoagulante. Variables principales: pulso (latidos/minuto) y CHA 2 DS 2 vascular. Resultados: se realizaron 789 mediciones de pulso en ambas campañas, de las cuales se encontró que 41 (5,2 %) participantes presentaban un pulso irregular. 35 (4,3 %) fueron derivados a sus médicos de familia, confirmándose 12 (1,5%) diagnósticos nuevos de FA e iniciando 7 (0,9 %) de ellos tratamiento anticoagulante. Más del 80 % de los participantes no presentaba ningún síntoma clínico de FA y un 41,9 % padecía HTA. El número de pacientes con CHA 2 DS 2 vascular ≥2 y pulso irregular fue de 17 (3,9 %), de los cuales 10 (58,8 %) estaban sin tratamiento; 5 (29,4 %) tenían tratamiento antiagregante y 2 (11,8 %) tratamiento anticoagulante. Conclusiones: tanto el elevado número de pacientes con FA diagnosticada y sin tratamiento como el número apreciable de pacientes con pulso irregular y sospecha de FA detectados demuestran la capacidad y eficacia de la farmacia comunitaria en la realización del cribado del pulso irregular contribuyendo así a la detección de nuevos casos de FA. A B S T R A C TIn the present article, we review publications from the previous year in the following 3 areas: clinical cardiology, geriatric cardiology, and heart failure and transplantation. Among the new developments in clinical cardiology are several contributions from Spanish groups on tricuspid and aortic regurgitation, developments in atrial fibrillation, syncope, and the clinical characteristics of heart disease, as well as various studies on familial heart disease and chronic ischemic heart disease. In geriatric cardiology, the most relevant studies published in 2014 involve heart failure, degenerative aortic stenosis, and data on atrial fibrillation in the geriatric population. In heart failure and transplantation, the most noteworthy developments concern the importance of multidisciplinary units and patients with preserved systolic function. Other notable publications were those related to iron deficiency, new drugs, and new devices and biomarkers. Finally, we review studies on acute heart failure and transplantation, such as inotropic drugs and ventricular assist devices.ß 2014 Sociedad Española de Cardiología. Entre las novedades en cardiología clínica se han incluido aportaciones de grupos españ oles en valvulopatía tricú spide y aó rtica, novedades en fibrilació n auricular, síncope y sobre las características clínicas de los cardió patas, así como varios trabajos sobre cardiopatías familiares y sobre cardiopatía isqué mica cró nica. Respecto a la cardiología geriá trica, los estudios má s relevantes durante el ú ltimo añ o está n centrados en insuficiencia cardiaca, estenosis aó rtica degenerativa y tambié n datos de fibrilació n auricular en la població n anciana. En insuficiencia cardiaca y trasplante, las novedades má s destacables se centran en la importancia de las unidades multidisciplinares, así como en los pacientes con funció n sistó lica preservada. Tambié n se destacan aportaciones en relació n con el dé ficit de hierro, los nuevos fá rmacos y los nuevos dispositivos y biomarcadores. Por ú ltimo se reseñ an trabajos centrados en insuficiencia cardiaca aguda y trasplante, como los fá rmacos inotró picos y los dispositivos de asistencia ventricular.
