Update on Gastrointestinal Radiopharmaceuticals and Dosimetry Estimates

Abstract: The gold standard technique for measuring gastric emptying is scintigraphy using radiolabeled test meals. Recently, a standardized radiolabeled solid meal has been proposed and adopted by many centers. There is still a need for alternative meals and several such meals with demonstrated radiolabel stability have been evaluated in small numbers of subjects. Updated radiation dosimetry associated with these meals has been calculated for adult males and adult females with normal GI transit as well as transit abnor… Show more

“…The test food used to measure gastric emptying must also meet additional criteria: the food must be in a solid form to stimulate the feeding stage and full physiologic gastric responses, should be palatable to encourage the subject to ingest the entire portion, able to effectively bind a radiolabeled substance that is not absorbed by the gastrointestinal (GI) tract, the portion size must be large enough to enable the differentiation between good and poor GI function and should be easily prepared and inexpensive to administer. 18 Bread, ham and rice have previously been used in conjunction with radiolabeled scrambled or microwaved egg as gastric emptying test foods. 6,19 To best clarify the relationship between masticatory performance and gastric emptying, the same test food should be used for both tests.…”

“…99m Tcsulfur colloid has a short half-life of 6 h and when properly cooked is physically bound to certain foods. Because it cannot be absorbed within the gastrointestinal tract, it causes only low radiation exposure (5). With a longer half-life of 67 h, 111 In-diethylenetriaminepentaacetic acid can be used to image gastrointestinal transit that requires 2-3 d, such as colonic transit.…”

“…Many of these methods are time-consuming, expensive, invasive, and some, such as scintigraphy, may involve significant levels of radiation exposure depending on the radioactive meal and clinical indication [2,4]. Furthermore, there are inter-investigational differences as to which anatomical area of the gut data are obtained from.…”
