Use of a gentamicin-impregnated collagen sheet (Collatamp®) in the management of major soft tissue complications in pediatric cochlear implants

Abstract: headings In all patients with soft tissue infection around the cochlear implant, infection was completely resolved. It was not necessary to remove the device in any case. The use of an absorbable gentamicin-impregnated collagen sheet is not described for the management of soft tissue complications in pediatric cochlear implant patients. The local application of high concentrations of antibiotic administered by this sheet may be effective against resistant bacteria and, in conjunction with surgery, may resolve … Show more

“…A recent study evaluated the effectiveness and safety of gentamicin sponges in soft tissue infections in paediatric CIs. In all three patients, the infection resolved after salvage surgery using Collatamp ® sponges with a follow‐up of 12 months . These infections consisted of serous fluid collections or haematoma without device exposure and therefore may be considered milder infections than our cases.…”

“…In all cases, any visible biofilm was mechanically removed, and gentamicin-impregnated collagen sheets were used to obtain a higher antibiotic concentration directly in the tissue surrounding the implant. 13,14 In the first patient, the Bonebridge had been implanted in the retro-auricular region using a classic pre-sigmoid approach. After oral antibiotic treatment, surgical debridement was performed, and a rotational skin flap was needed to close the defect (Figure 1).…”

“…The administration of antibiotics like rifampicin has shown higher efficacy in treating biofilm-associated infections, particularly against S. epidermidis and Methicillin-resistant Staphylococcus aureus (MRSA) ( Im et al, 2015 ). The use of absorbable gentamicin-impregnated collagen sheets has a bactericidal effect on Gram-positive bacteria, including Staphylococci , making it a potential adjunct treatment for soft tissue infections ( Benito-González et al, 2014 ). Long-term, low-dose administration of clarithromycin (500 mg for 24 h in adults) has been found to inhibit protein synthesis and effectively kill P. aeruginosa ( Garcia‐Valdecasas, Jiménez‐Moleon, Sainz, Fornieles, and Ballesteros, 2009 ).…”
