Use of Aortic Valvo-Pumps Placed in Valve Annulus for Long-Term Left Ventricular Assist

Abstract: To provide better anatomical fit and physiologic adaptation, three aortic valvo-pumps with different dimensions were developed. Each pump has a rotor with an impeller and drive magnets and a stator consisting of a motor coil with iron core and an outflow guide vane. The devices had outer diameters of 21 mm, 23 mm, and 25 mm, respectively, and weighted 27 g, 31 g, and 40 g, respectively. Laboratory testing demonstrated that the rotating speed for maintaining a diastolic pressure of 80 mm Hg at zero flow rate sh… Show more

“…These have been attributed to continuous aortic valve closure during cf-LVAD support. Moreover, the lack of pulsatility and the absence of aortic valve movement with cf-LVAD support have also been linked to the increased incidence of gastrointestinal bleeding in these patients (4,5). Therefore, some centers have strived to achieve intermittent aortic valve opening in their LVAD patients (6).…”

“…Qian et al. have also developed a third‐generation axial flow pump that can be implanted at the aortic valve position . Our latest axial flow blood pump, the third‐generation blood pump, consists of a direct‐drive impeller, a hybrid bearing including a passive magnetic bearing and a hydrodynamic bearing .…”

“…Therefore, these three pumps may meet the hemodynamic requirements of patients with body weights of 40–60 kg, 60–80 kg, and 80–100 kg, respectively. The devices occupy no additional anatomical space and deliver the blood directly from the ventricle to the aorta, thus producing less physiological disturbance to the natural circulation (25,26). In 2005, the first in vivo trial of this device demonstrated that the 25‐mm aortic valvo‐pump can be sewn onto the aortic valve annulus of an 80‐kg pig without injury to adjacent tissues and organs (27).…”

“…For long-term left ventricular assist, aortic valvo-pumps were developed [1][2][3][4]8], which deliver the blood directly from ventricle to aorta without connecting tubes and bypass circle, thus have less dangers of thrombus formation outside the pump. In the inside of the pump, regurgitation can wash out the blood contacting surfaces in the gap between the rotor and the stator and can prevent the formation of micro-thrombus thereafter.…”
