Use of Environmental Manipulation and Modified Informal Reality Orientation With Institutionalized, Confused Elderly Subjects: A Replication

Abstract: The effectiveness, for confused elderly subjects, of modified informal reality orientation and environmental manipulation without any 'classroom' reality orientation was investigated with 10 experimental subjects in one ward and 10 control subjects in another. All subjects were assessed prior to the start of the intervention and after 6 and 12 weeks of treatment on cognitive and behavioural measures. While the control group showed decline on all measures except one, the experimental group improved in ward orie… Show more

“…Originalmente, el énfasis estaba centrado en administrar al paciente información a cualquier oportunidad. Más recientemente, se ha desarrollado una aproximación más ecológica en la que el staff responde directamente a las preguntas o dudas de los pacientes o les ayuda a descubrir las respuestas por ellos mismos 6,7 . Esta variante probablemente tenga menos riesgo de sobreestimulación para el paciente que cuando se le proporciona información que no es capaz de asimilar.…”

“…There is a substantial literature on reality orientation (RO) (see Holden and Woods, 1995, for a detailed review). Studies by Reeve and Ivison (1985) and Williams et al (1987) indicate that the introduction of a form of 24-hour RO was associated with improvements in general behavioural function as well as more speci®cally in ward orientation. More speci®c training appears to be needed to in¯uence behaviour and function.…”

“…Both the more recent RO research literature (Baines, Saxby & Ehlert, 1987;Williams et al, 1987) and the literature on practice concepts (Davis & Kirkland, 1988;Edelson & Lyons, 1985;Fabiano, 1987;Holden & Woods, 1982;Judd, 1983;Zgola, 1987) reflect an evolution of the approach to reality orientation programming. For example, RO groups as described by Holden & Woods (1982) include meaningful activities, sensory rich experiences, music, structured reminiscence, familiar clues and structured socialization, and emphasize success experiences and adaptability of approach.…”

“…For example, RO groups as described by Holden & Woods (1982) include meaningful activities, sensory rich experiences, music, structured reminiscence, familiar clues and structured socialization, and emphasize success experiences and adaptability of approach. The preliminary research on these groups (Baines, et al 1987;Williams, et al 1987) suggests that they positively affect not only verbal orientation, but also other functional behaviours such as communication and activities of daily living. This reformulated approach is taking hold especially in Britain and the conventional RO classroom "cookbook" approach is falling by the wayside.…”
