Use of Environmental Manipulation and Classroom and Modified Informal Reality Orientation With Institutionalized, Confused Elderly Patients

Abstract: The effectiveness of 'classroom' reality orientation (CRO) for confused elderly subjects was evaluated. Experimental groups 1 and 2 (each consisting of five subjects) received CRO for 1 month with only group 1 continuing with maintenance CRO for a further 2 months. Both experimental groups and the control group (10 subjects) received modified informal reality orientation (IRO) and environmental manipulation (EM) for all 3 months. All subjects were assessed prior to the start of the study and after 4 and 12 wee… Show more

“…Baines et al (1987) suggest that positive outcomes can be maintained to a degree when an intervention has ended, and lost gains can be retrieved when a further intervention is put into place. Reeve and Ivison (1985) demonstrated that effects following RO were maintained, at least for a limited period, although this was enhanced by further input. In this study, a group of people diagnosed with dementia received classroom RO for a month, and significant differences on cognitive and behavioural outcome measures were found in comparison to a control group.…”

“…Returning to the study by Reeve and Ivison (1985), there are a number of problems that must be considered. One is that all three groups (the treatment group receiving continued input, the treatment group that did not receive continued input and the control group) received what was classified as 'modified informal reality orientation' and 'environmental manipulation' throughout the course of the study and therefore were all actually receiving some form of intervention.…”

“…Originalmente, el énfasis estaba centrado en administrar al paciente información a cualquier oportunidad. Más recientemente, se ha desarrollado una aproximación más ecológica en la que el staff responde directamente a las preguntas o dudas de los pacientes o les ayuda a descubrir las respuestas por ellos mismos 6,7 . Esta variante probablemente tenga menos riesgo de sobreestimulación para el paciente que cuando se le proporciona información que no es capaz de asimilar.…”
