Use of Occlusal Splint for Noninvasive Fixation of a Reference Frame in Orbital Navigation Surgery

Abstract: Introduction When prepping for navigation surgery, a reference frame must be fixed to the patient's head with a Mayfield clamp, bone anchor, headset, or headband. Fixation of the clamp or bone anchor with a screw or head pins is invasive, whereas use of a headband or headset is noninvasive. However, during orbital surgery for blowout fractures or orbital tumors, surgeons or instruments can interfere between the reference frame and the optical tracking navigation system, even if using noninvasive fixation. Mate… Show more

“…However, the soft tissue could shift and deform during the operation, which will greatly affect the precision of these two methods. Occlusal splints provide a noninvasive, highly accurate registration method that is steadily fitted to stable bony landmark-dental cups 2325 . Due to its advantages, this method has been widely used in cranio-maxillo-facial surgical navigation.…”

“…With this, the occlusal splint can provide a noninvasive, high-precision registration method, because it can be stably installed on the teeth with bony structures. 32 , 37 , 38 Therefore, we use the occlusal relationship of the teeth to make the fiducial markers tightly integrated with the skull. Cutting et al 39 studied the use of sharp bony marks for registration in AR systems, that is, through the bite plate positioning mark, the final control error is less than 1.5 mm, and the rotation error of any axial position is <3°.…”

“…Moreover, the irregular shape of the teeth makes it easy to form a mosaic state with other materials. With this, the occlusal splint can provide a noninvasive, high-precision registration method, because it can be stably installed on the teeth with bony structures 32,37,38 . Therefore, we use the occlusal relationship of the teeth to make the fiducial markers tightly integrated with the skull.…”

“…Several studies have proposed designs of a Dynamic Reference Frame supported by a splint 20 – 31 . This position of the Dynamic Reference Frame may be less invasive than fixation on the patient’s cranium 32 . Registration outside the patient has been proposed, both with a fiducial-based 20 23 , 25 27 and automatic method 28 – 31 .…”
