Using A Video-Based Advance Care Planning (ACP) Website to Facilitate Group Visits for Diverse Older Adults in Primary Care Is Feasible And Improves ACP Engagement (TH307D)

Abstract: Conclusion. The purely patient-facing, ACP PREPARE website plus an easy-to-read advance directive compared to a directive alone increased ACP documentation and engagement among diverse older adults.
