Uterine didelphys associated with obstructed hemivagina and ipsilateral renal anomaly (OHVIRA) syndrome

Abstract: Obstructed hemivagina and ipsilateral renal anomaly (OHVIRA) syndrome is a rare complex of structural abnormalities of the female urogenital tract. A 17-year-old girl with uterine didelphys associated with OHVIRA syndrome presented with progressive development of cyclic lower abdominal discomfort and a large abdominopelvic mass. We describe the findings from ultrasound, computed tomography (CT), and magnetic resonance imaging (MRI), the first case report of this syndrome to examine all three different imaging … Show more

“…Uterine anomalies can result from failure of the Müllerian ducts to fuse in the midline, to connect with the urogenital sinus, or to create the appropriate lumen in the upper vagina and uterus by resorption of the central vaginal cells and the septum between the fused Müllerian ducts [ 2 4 ]. Lack of fusion of the two Müllerian ducts results in duplication of corpus and cervix [ 5 ]. Occasionally, uterus didelphys is associated with an obstructed hemivagina and often with ipsilateral renal agenesis [ 6 ].…”

“…In case of septate uterus, septa is demonstrated unequivocally by MRI. [7][8][9][10] The standard management of these patients is excision of vaginal septum and drainage of hematometrocolpos. 9,10 …”

“…Ultrasonography is frequently used as initial investigation and preferred as it is non-invasive, free of radiation, and widely available but cannot identify types of MDA. [28][29][30] Magnetic resonance imaging is ideal imaging modality for noninvasive evaluation of female pelvic anatomy because it provides more detailed information regarding uterine morphology, continuity with each vaginal lumen, character of vaginal septum, and nature of fluid contents. It is more useful in detection of associated conditions, such as, endometriosis, pelvic inflammation and adhesions, and coexisting urologic anomalies.…”
