Objective: To compare the results of operations of commercial uterine manipulators. Considering that the optimal uterine manipulator is still not found, our goal was to give an idea for gynecologists to choose the most suitable uterine manipulator for their purposes and expectations. Methods: Between January 2016 and September 2021, 294 laparoscopic hysterectomy cases met the inclusion criteria and were operated in İzmir Katip Çelebi University Atatürk Training and Research Hospital Gynecology and Obstetrics Clinic. They were divided into four groups according to the type of manipulator used. Group-1 (RUMI-I), Group-2 (Clermont-Ferrand), Group-3 (sharp intrauterine curette-tenaculum), and Group-4 (without uterine manipulator). All four groups were compared with operation time, hospital stay, absolute change in hemoglobin (g/dl), and per-op complications. Results: Considering the operation times in all groups, the mean operation time in the first Group-was 180 minutes (98-349); in Group-2 was 159 minutes (96-564); in the 3rd Group was 178 minutes (141-540); in the 4th Group was 189 minutes (115-453). The group with the shortest operation time was Group-2; the difference was statistically significant (p=0.015). In general terms, the effect of all manipulators on patient parameters wasn’t very different. Clermont-Ferrand seems more advantageous than others in making the surgeon’s work more straightforward regarding operation time only. Conclusions: In our study, the choice of uterine manipulator did not affect the surgical results except for the duration of the operation. Abdominal pain is a very common presentation in early pregnancy. Its cause may be gynecological or totally nonpregnancy related. While acute appendicitis is the most common nonobstetric cause of pain in pregnant women, diagnosis and differentiation from other causes, including ectopic pregnancy, remain challenging. In clinical situations of uncertainty, laparoscopy is a useful diagnostic tool, but uterine manipulation should be avoided if an intrauterine pregnancy is a possibility. In this report, we describe a case of complicated appendicitis in very early pregnancy where the patient ended with a full-term healthy pregnancy despite undergoing a diagnostic laparoscopy with inadvertent uterine manipulation.

Background: The purpose of this study was to explore the value of comprehensive nursing intervention in the perioperative period of ruptured bleeding of ectopic pregnancy.Methods: From January 2015 to January 2020, 164 patients with rupture and bleeding of ectopic pregnancy who needed laparoscopic treatment in the department of gynecology at our hospital were selected and randomly divided into the basic nursing group and the comprehensive nursing group, with 82 cases each.During the perioperative period, comprehensive nursing intervention or basic nursing intervention were performed, and the nursing effects of the two nursing interventions were compared. Results:The disappearance time of abdominal pain, the time to get out of bed, and the length of hospitalization in the comprehensive nursing group were significantly shorter than those in the basic nursing group (P<0.05). After surgery, blood sugar levels, aldosterone, cortisol, C-reactive protein (CRP), and IL-6 in the two groups were significantly higher than those before surgery (P<0.05), but there was no statistically significant difference between the groups (P>0.05). After the operation, the proportion of patients with Visual Analogue Scale (VAS) scores of 7-10 in the comprehensive nursing group was significantly lower than that in the basic nursing group (P<0.05). Before the intervention, the Hamilton Anxiety Scale (HAMA) and Hamilton Depression Scale (HAMD) scores of the two groups were compared, and the difference was not statistically significant (P>0.05). After the intervention, the HAMA and HAMD scores of the comprehensive nursing group were significantly lower than those of the basic nursing group (P<0.05). The fallopian tube recanalization rate of patients in the comprehensive care group was significantly higher than that of the basic care group (P<0.05), and the complication rate was significantly lower than that of the basic care group (P<0.05).Conclusions: In summary, a comprehensive nursing program during the perioperative period can improve the treatment effect and significantly shorten the recovery time of patients, which is worthy of clinical promotion.
