Background Fluorescence in situ hybridization (FISH) is highly accurate in detecting specific genetic rearrangements or gene amplifications. It provides direct visualization of chromosomal abnormalities or gene fusions at the cellular level.Objective This article investigated the molecular labeling of maxillofacial lesions and examined the detectable genetic alterations using FISH analysis. The study discussed the merits and limits of FISH analysis in diagnosing these lesions and provided insights into the future applicability of FISH as a diagnostic tool. MethodWe conducted a comprehensive search of electronic databases (e.g., PubMed, Embase, Web of Science) to identify relevant studies published within a specific time frame. We utilized appropriate search terms, including "maxillofacial lesions," "fluorescence in situ hybridization," "genetic alterations," "diagnosis," and related keywords .Included articles published in English, targeting human subjects, and focused on the molecular characterization of maxillofacial lesions using FISH analysis. Iterative refinement and validation were used for improving the accuracy of predictions of the timeseries analysis. ResultsWe identified the limitations and challenges associated with FISH analysis in maxillofacial lesions, such as the non-specificity of genetic alterations and the limited availability of fusion probes. We analyzed the implications of these findings and inferred the future applicability of FISH analysis in the diagnosis of maxillofacial lesions. Advances in KnowledgeThe evolving landscape of molecular diagnostic techniques uses more trendy and efficient techniques than FISH analysis. With competing such techniques, FISH is less likely to be of use soon unless promising avenues are to be proposed for overcoming existing limitations.

The study aimed to compare 15 cases of mucoepidermoid carcinoma (MEC) and 15 cases of hyalinizing clear cell carcinoma (HCCC) using immunohistochemical staining and molecular analysis. Thirty samples were examined, and markers, including p63, CK5/6, SOX10, CK7, ATF1, and FISH probes specific to EWSR1 and MAML2, were used. Clear cell differentiation was observed in all MEC cases to some extent, with clear cell MEC showing the most prominent findings. Clear cell features were also present in conventional MEC, oncocytic MEC, and Warthin-like MEC, although to a lesser extent. The majority of cases were classified as low-grade MECs. MAML2 rearrangement was detected in all cases (except cases 11 and 14), while EWSR1 rearrangement was observed in a single case of clear cell MEC. These findings helped identify distinct subtypes within the mucoepidermoid carcinoma spectrum. The study emphasized the importance of utilizing immunohistochemical profiles, histopathological features, and molecular analysis for accurate diagnosis and classification of salivary gland neoplasms. HCCC was also discussed, and ATF1 was proposed as a marker to distinguish HCCC from morphologically similar neoplasms. The study concluded that a comprehensive approach combining immunohistochemistry, histopathology, and clinical correlation is essential for accurate diagnosis and classification, considering the variable expression of markers and potential overlap with other tumor types.

The Milan System for Reporting Salivary Gland Cytopathology (MSRSGC) was introduced into cytopathology practice more than 5 years ago. It classifies the salivary gland lesion into 6 diagnostic categories and provides the risk of malignancy (ROM) and clinical management guidelines for each category. More than 100 articles have confirmed the applicability of this reporting system in routine practice and its important role in providing a uniform reporting system for salivary gland fine-needle aspiration. At the same time, new questions and feedback for improvement have emerged, as well as opportunities for clarification. For example, questions related to the non-diagnostic category are multiple-fold. First, although the cytologic criterion of the non-diagnostic category is currently defined as "<60 lesional cells or normal salivary gland tissue within the clinical setting of an evident mass", this has not been established or validated in the literature. Second, the ROM for the non-diagnostic category is high. Another question surrounds the interesting topic of sub-classifying current MSRSGC categories, as the risk of malignancy could vary in tumors of the same category. The last one concerns the incorporation of the ever-increasing number of molecular markers and antibody detection of gene re-arrangements, so-called next-generation immunohistochemistry (IHC) markers, into routine cytopathology practice. The quick application of nextgeneration sequencing into pathology practice provides an exciting opportunity for salivary gland cytopathology diagnosis.
