Vaginal Misoprostol vs Vaginal Misoprostol With Estradiol for Labor Induction: A Prospective Double Blind Study

Abstract: Objective To compare the safety and effectiveness of vaginal misoprostol with combined vaginal misoprostol and estradiol for induction of labor in unfavorable cervix. Method A prospective study was carried out from Jan 2008 to Jul 2008 on total of 90 women with unfavorable cervix (Bishop's score was \5) and gestation [36 weeks with clinical indication for induction of labor. They were randomly assigned to receive either vaginal misoprostol 25 lg alone or vaginal misoprostol 25 lg with vaginal estradiol 50 lg. … Show more

“…Therefore, it has been argued that DHEAS is metabolized to estrogens in term cervix, which in turn initiates CR. However, studies examining such effects showed that administration of estrogens alone fails to ripen the cervix ( Thiery et al , 1979 ; Larmon et al , 2002 ; Dasgupta and Singh, 2012 ). Notably DHT, which cannot be metabolized to estrogens, also promotes CR, implying an androgen-specific effect ( Ji et al , 2008 ).…”

“…Also similar to the present study, RU486 treatment of pregnant rats induced preterm birth within 28 h as serum estradiol increased 2-fold [40] . The possibility remains that estradiol may act synergistically to facilitate the remodeling process [41] or antagonize progesterone actions [42] . However, differences in preterm remodeling characteristics between Ovx and other groups in this study direct attention to the possibility that the cervix maybe sufficiently remodeled in the rat by 3 or more days before term when serum progesterone is elevated and comparable to near peak concentrations in previous studies [7] , [9] , [14] , [43] .…”

“…In all three groups, the evaluation of the caesarean indication was similar, including failure to progress and meconium-stained liquor. Like studies by Souizi et al (7%), Dasgupta and Roudsari et al (10%), the caesarean rate was 7% [ 21 , 31 , 32 ]. No statistically significant differences have however been reported.…”
