Validity assessment of self-reported smoking status in firefighters using the urine cotinine test

Abstract: Background: In firefighters, smoking management is important because they are exposed to various harmful substances in their occupational environment. Accurate surveys of smoking status are essential to control tobacco use. The main disadvantage of self-report questionnaires, which are commonly used for investigating smoking status, is the possibility that the subjects' response are invalid. If the validity of firefighters' answers on smoking questionnaires is not adequate, different methods will be needed for… Show more

“…Genetics, lifestyle, and other health factors might all come into play [ 37 ]. In addition, because smoking is generally a socially and medically stigmatized behavior, smokers tend to underreport their smoking status [ 39 ]. Therefore, self-report questionnaires may have limitations in determining smoking status.…”

“…One possible explanation for why the hip fracture risk was not significantly higher in older current smokers compared to never-smokers is that the effect of smoking on bone density may be attenuated by other factors that are more prevalent or influential in old age, such as osteoporosis, frailty, comorbidity, medication use, or nutritional status [ 37 ]. Another possible explanation is that the survey used to record smoking amounts and duration might not perfectly reflect the actual time someone has smoked [ 39 ]. People might not remember accurately or might change their smoking habits over time.…”

“…First, this survey was based on a self-reported questionnaire. Therefore, there might be an underestimation of exposures [ 28 ]. Also, the outcome was defined as visits to the dentist for the treatment of periodontal disease; therefore, localization, severity, or subtypes of the disease could not be discerned.…”

“…2 On top of wide-spread use, dishonesty in selfreported nicotine use is common making cessation studies difficult to conduct. [3][4][5][6] Gold-standard analyses to objectively report use exist to aid these cessation studies (eg. HPLCtandem mass spectrometry), however, they are expensive (≥$47 per test) and immobile, making them not suitable for high-throughput testing.…”
