Value of CD23 Determination by Flow Cytometry in Differentiating Mantle Cell Lymphoma From Chronic Lymphocytic Leukemia/Small Lymphocytic Lymphoma

Abstract: Mantle cell lymphoma (MCL) and chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL/SLL) share many morphologic and immunophenotypic features. In addition to histomorphologic examination, it is customary to use the absence of CD23 to differentiate MCL from CLL/SLL, based primarily on reported comparisons of immunohistochemical staining of tissue sections. These findings are widely extrapolated to flow cytometric analysis, although available data are contradictory and not sufficiently detailed. We compa… Show more

“…CD23 dim positivity has already been reported in MCL. 45,46 Even if its expression appeared more frequent in the non-nodal group, it was always dim and significantly lower than that observed in CLL. Although median survival appeared longer in the nonnodal group (41 months) than in the nodal group (29 months), the overall survival was not significantly different by univariate analysis, nor was it selected by multivariate analysis.…”

“…16,17 It should be noted that the curve was partially overlaid with the isotype control, and the ratio of the MFI value to the isotype was 2.0, indicating a relatively low expression level on MCF-7 cells. 23 Multidrugresistant MCF-7/ADR cells were used as a positive control, 24 and the ratio of the MFI to isotype for these was 7.0, much higher than for the MCF-7 cells. HeLa was a P-gp-negative cell line, and there was no significant difference between its MFI value and that of its isotype (3.75 vs 3.36).…”

“…Not only can MCL be difficult to distinguish from CLL morphologically, but the neoplastic cells similarly express CD5. However, in contrast to CLL, MCL is usually CD23 Ϫ , FMC7 ϩ , and CD79b ϩ (1,7,17,18), although dim CD23 ϩ staining may be seen (18). MCL is more often aggressive than CLL and patients may benefit from earlier and different therapeutic approaches (19 -22).…”
