Vemurafenib-Induced Eccrine Squamous Syringometaplasia

Abstract: We report herein a patient treated with vemurafenib for a stage IV melanoma who developed an eruption related to eccrine squamous syringometaplasia (ESS). This entity is a well-described side effect of cytostatic therapies used for malignant neoplasia and is clinically characterized by a symmetric cutaneous eruption composed of papules and vesicles preferentially located on fold and intertriginous areas. It is histologically defined by a squamous metaplasia of eccrine ductal epithelium. ESS represents another … Show more

“…In these cases, there was a very predominant eccrine squamous syringometaplasia (ESS) associated with the histological features of NEH resulting from damage to the sweat glands. ESS has been reported in the context of BRAFi‐treated patients with melanoma . These cases raise the possibility that it might represent a later stage of NEH more than direct drug toxicity.…”

“…Although the specific cellular mechanisms to induce eccrine syringometaplasia have not been identified, it has numerous etiologic associations. Inflammation is the most common association, but it has also been linked to herpes virus infections and several chemotherapeutic drugs, most recently BRAF inhibitors vemurafenib and dabrafenib [9,11,12,13,14,15]. Based on 7 patients with a mean follow-up of 10 years, Brown et al [16] postulated that primary FM is a clonal disorder with limited or benign cutaneous manifestations.…”

“…Keeping this in mind, it is important to investigate the concentration of cytostatics in sweat as a noninvasive procedure for subjects. This is significant not only for the estimation of drug treatment efficacy, but also for the explanation of some skin-related side effects, such as Grover's disease, which appears as a consequence of the concentration of chemotherapy drug metabolites in sweat [25,26]. However, the above-mentioned solutions demand complex and costly technological processes for manufacturing of the whole device, and the systems are not based on textile materials.…”
