Verapamil in atrial fibrillation and atrial flutter

Abstract: A double-blind randomized study was performed to compare the efficacy of intravenous verapamil with saline in 28 patients with a rapid ventricular rate and atrial fibrillation or atrial flutter. Conversion of atrial fibrillation to sinus rhythm occurred in none of 14 patients after saline and in 3 of 20 patients (15%) 7 to 160 min after verapamil. The ventricular rate in atrial fibrillation was slowed greater than or equal to 15% in 2 of 14 patients (14%) by saline, in 17 of 20 patients (85%) by 1 dose of vera… Show more

“…Verapamil was successful in 8-15% of patients. [14][15][16] Duration of fibrillation10 and the size of the left atrium' " influence the success of conversion. All the previously mentioned studies were largely performed in subjects with non-established paroxysmal or acute atrial fibrillation.9 16 Although the duration of fibrillation varied between studies, making comparison of the therapeutic effects difficult, fibrillation had not lasted more than one day in most patients.…”

“…As part of another protocol to assess efficacy of verapamil in long-term control of ventricular response, ten patients who had control but did not convert were begun on a regimen of oral verapamil in a dosage of 80 mg every six hours, of which three (patients 7, 9, and 12) converted to sinus rhythm within 24 hours of beginning administration. These patients all had atrial flutter of relatively short duration (one month, 17 hours, and 21 days, respectively). Figure 1 shows the response of MAP to the IV infusion of a single dose of verapamil in nonconverters.…”

“…Its use in the treatment of AF with a high ventricular rate is worldwide [28][29][30][31][32][33][34][35][36]. Recently [37,38] we have employed verapamil in 139 patients with AF of different etiologies.…”
