Vernarbende Tinea profunda des Mons pubis durch Trichophyton verrucosum

Abstract: A 22-year-old female veterinary student developed a severe inflammatory fungal infection of the pubic area 2 months after practical training in obstetrics at a cattle farm. Fungal culture revealed a slowly growing white dermatophyte, first differentiated as Trichophyton mentagrophytes. Because of the unusual location, molecular species differentiation was performed identifying Trichophyton verrucosum. Although the patient was treated with systemic antifungals, the infection resulted in scarring alopecia.

“…In the patients presented here a longer treatment period was necessary (mean 5 weeks). In the recent literature, an example of prolonged therapy was described in a case report of a female veterinary student with a severe inflammatory fungal infection of the pubic area after working in a cattle farm 20. In that case, the pathogen was identified as T. verrucosum , also a zoophilic dermatophyte.…”

“…Die Möglich-keit einer Transmission der genitalen Infektion über sexuelle Kontakte muss bedacht werden [13]. Nicht zuletzt durch die meist zu spät initiierte orale antimykotische Behandlung kann es zu narbiger Abheilung im Sinne einer Pseudopelade Brocq kommen [7].…”

“…Although several secreted proteases from dermatophytes have been obtained as recombinant proteins, no T. verrucosum-secreted proteases are currently available in recombinant form. In spite of their different ecologies, the genome of T. rubrum (21) proved to be most closely related to the genomes of T. verrucosum and Arthroderma benhamiae, two phylogenetically closely related dermatophytes that induce highly inflammatory cutaneous infections in humans (22,23). In addition, most Trichophyton species revealed similar secreted protein profiles (24).…”
