Very Late Metastasis (27 Years) of Cutaneous Malignant Melanoma Arising in a Halo Giant Congenital Nevus

Abstract: We report a case of primary cutaneous melanoma with the incidental finding of a lung metastasis 27 years following the original diagnosis. The case is exceptional in that it is a late metastasis of a melanoma that arose in association with a halo giant congenital nevus. The original tumor was a large dermal/subcutaneous nodule composed of very well-differentiated cells reminiscent of type B nevomelanocytes. The metastasis displayed similar histology. This case emphasizes the unpredictable behavior of malignant… Show more

“…Not all melanomas are lethal during the lifetime of a patient, and some metastasize many years, even decades, after the removal of the primary skin tumor. 11 If one accepted ASMTs with positive SLNs as melanomas, a finding in keeping with a less aggressive clinical course would be that the majority of lymph node tumor deposits were small. Small tumor volume in a SLN has been associated with better prognosis even for conventional adult-type melanoma.…”

“…Halo naevi have occurred in patients with melanoma as well as in patients w ith melanoma in giant CMN (32,33). However, a giant CMN perse carries a considerable risk of malignant transformation (34).…”

“…CMN with a halo phenomenon have infrequently been associated with melanoma. 27,30 As with all CMN with unusual features, continuous observation is prudent. 26 Three patients in our study had a history of vitiligo.…”
