Very long-term results (up to 17 years) with the double-orifice mitral valve repair combined with ring annuloplasty for degenerative mitral regurgitation

Abstract: The double-orifice repair combined with ring annuloplasty provides very satisfactory long-term results in patients with degenerative mitral regurgitation in the setting of bileaflet and anterior leaflet prolapse.

“…As far as the EE technique is concerned, in patients with degenerative MR, the long-term results of this procedure are excellent whenever an annuloplasty is concomitantly used. 7,8 However, when annuloplasty is omitted and annular dilatation is not corrected, computational models have predicted higher stresses on the suture and on the leaflets, possibly leading to progressive degeneration of the reconstruction. 9 In the clinical setting, we first reported a higher failure rate at 5 years for the EE technique without ring annuloplasty in 260 patients submitted to the double-orifice repair.…”

“…In this series, the degree of acute residual MR was a predictor of follow-up MR >2þ recurrence (p ¼ 0.02), 24% of patients being discharged with 2þ residual MR. Indeed, in the surgical experience, a suboptimal repair with residual prolapse and 2þ MR is a well-known predictor of recurrent severe MR. 15 MitraClip repair is a leaflet procedure and lacks annuloplasty, which is fundamental to achieve optimal and durable results in surgery. We recently published our long-term results with the edge-to-edge technique without annuloplasty in the surgical setting, 16 reporting a disappointing freedom from MR recurrence of 64% at 10 years despite an initial optimal result (MR 1).…”

“…A limitation of this technique is that the cause of the annulus‐dilatation in FMR is not addressed by this technique. OMVR with E‐t‐E repair combined with an annuloplasty ring provides an increased long‐term durability and decreased risk of recurrent MR . Long‐term durability of the technique without an annuloplasty ring has not yet been proven and the number of patients treated this way are limited in comparison with other oMVR techniques.…”
