Vestibular Evoked Myogenic Potentials in Ipsilateral Delayed Endolymphatic Hydrops

Abstract: We recorded vestibular evoked myogenic potentials (VEMPs) in 12 patients diagnosed as having ipsilateral delayed endolymphatic hydrops (DEH). Seventy-five percent (9/12) of the patients showed decreased or absent VEMPs in the affected ears. Almost all patients had normal VEMPs in the unaffected ears. In addition, in 4 patients, VEMPs were recorded before and 3 h after oral glycerol administration (1.3 g/kg body weight). VEMPs improved after glycerol administration in 2 of the 3 patients whose VEMPs had been ab… Show more

“…Since VEMPs have been shown to reflect the function of the saccule and the inferior vestibular nerve [10][11][12] , those patients who showed abnormal VEMPs might have such pathologies in the saccule. Our finding that 61% of the patients with DEH showed abnormal VEMPs is consistent with previous studies [18][19][20] . Young et al [18] recorded VEMPs in 20 patients with DEH, and reported that 55% of the patients showed abnormal VEMPs on one side.…”

“…Young et al [18] recorded VEMPs in 20 patients with DEH, and reported that 55% of the patients showed abnormal VEMPs on one side. Ohki et al [19,20] reported that abnormal VEMPs were observed in 75% of patients with ipsilateral DEH and 67% of patients with contralateral DEH. In the present study, 73% of patients with ipsilateral DEH and 43% of patients with contralateral DEH showed abnormal VEMPs on one side.…”

“…Absence of response to Vestibular evoked myogenic potentials in cases of Ménière's disease suggests saccular hydrops 8,10,12 . Depending on the severity of hydrops, some patients may present irreversible degeneration of sensorial epithelium of saccular macula 5,6,8,13 . Absence of response on the non-affected ear was found in 20% of our cases.…”

“…This probability was scored subjectively based on the ratio of the summating potential to the action potential (SP/AP) on an electrocochleogram, 15,16 the vestibular evoked myogenic potential (VEMP), 17 audiogram frequency and ‰uctuation patterns, and clinical history. A positive VEMP response is considered to be a normal sign of vestibular function, especially for the saccule.…”
